摘  要:目的:探讨股骨颈疝窝的影像学表现。方法:回顾性分析73例具有典型影像学征象的股骨颈疝窝的影像学资料。所有病例均行CT扫描,其中10例行MRI检查,16例行X线平片检查。结果:73例中18例为双侧股骨颈受累,91侧受累股骨颈中17侧见2个或2个以上病灶,73例共发现117个病灶。病灶均位于股骨头颈交界区前外侧皮层下,呈圆形、卵圆形或轻度分叶,病灶最大径2~11mm。平片上病灶表现为伴清晰薄层硬化缘的透亮区。CT上病灶周围均有环状硬化,其中62.4%(73/117)呈软组织密度、19.7%(23/117)呈液性密度、17.9%(21/117)呈脂肪密度,41.0%(48/117)在病灶或上下相邻层面上前方皮质显示与病灶相通的局部裂隙状缺损。MRI上多数病灶于T1W上呈低信号,T2W上呈被低信号环绕的高信号。结论:依据病变典型部位、形态及特征性影像学表现可确定股骨颈疝窝的诊断。Objective:To describe the imaging features of herniation pits of the femoral neck.Methods:Imaging findings of 73 patients with herniation pits of the femoral neck diagnosed by clinical imaging were analyzed retrospectively.All cases were examined by CT, 10 cases by MRI, 16 cases by conventional radiography.Results: 117 herniation pits of the femoral neck were found in 73 cases,of which bilateral femoral necks were involved in 18 cases and more than one lesion were found in 17 femoral necks of 15 patients.The lesions were round,oval or lobular,located in the superior lateral quadrant of the femoral head-neck junction.Lesions were 2-11mm in greatest diameter.Conventional radiographs revealed radiolucent lesions surrounded by a thin clear sclerotic rim.CT displayed well-defined lesions with sclerotic margins in all lesions.62.4%(73/117) of lesions were soft tissue density, 19.7%(23/117) revealed fluid density,and17.9%(21/117) displayed fat density.41.0%(48/ 117)of the lesions had focal cortical perforation on CT.T1-weighted images revealed low-signal intensity.T2- weighted images displayed central high signal intensity bounded by a dank peripheral ring.Concluslon:On the basis of its characteristic location,morphologic findings,and imaging appearance allow identification of herniation pits of the femoral neck.
