摘 要:目的探讨青少年特发性脊柱侧凸(adolescent idiopathic scoliosis,AIS)女性患者骨组织中I型胶原、骨钙素和骨保护素mRNA含量的变化与骨量减低之间的关系。方法43例AIS女性患者,年龄12~18岁。Cobb角范围40°~105°,平均50.3°±12.8°。将所有AIS患者分为两组:A组为骨量正常患者,B组为骨量减低患者。运用RT-PCR方法检测术中取髂骨松质骨块中I型胶原、骨钙素和骨保护素mRNA的表达。结果本组病例中骨量正常者(A组)29例,而骨量减低者(B组)14例(32.6%)。两组患者的生长发育相关资料等无显著性差异。两组患者间骨钙素和骨保护素mRNA的相对含量无显著性差异,而A组患者I型胶原mRNA的相对含量高于B组患者,但无统计学意义(F=1.761,P=0.196)。相关分析显示全部患者的I型胶原相对含量与BMD值之间呈较弱的正相关性,骨钙素和骨保护素与BMD值无显著相关性。结论本组病例AIS女性患者的松质骨中I型胶原、骨钙素和骨保护素等的mRNA表达量与骨量减低缺乏显著关联。不过,AIS女性患者的骨量减低与松质骨中I型胶原含量之间的关系难以排除。Objective To investigate the relationship between mRNA expressions of type Ⅰ collagen, osteocalcin and osteoprotegerin in cancellous bone and the decrease of bone mineral density in girls with adolescent idiopathic scoliosis (AIS). Methods This study involved 43 girls with AIS, aging from 12 to 18yr, with a mean Cobb angle of 50.3 + 12.8 ° (range 40° - 105 °). They were divided into two groups. Patients in group A maintained normal bone mineral density (BMD). Patients in group B sustained osteopenia. Small cancellous bone samples were harvested from the iliac crest during the operation. Total RNA was extracted, and reverse transcription-polymerase chain reaction Ⅰ collagen, osteocalcin and osteoprotegerin. Results (RT-PCR) was used to assay mRNA expressions of type There were 29 and 14 patients in group A and group B, respectively. Osteocalcin and osteoprotegerin mRNA contents were similar in both groups. No significant correlation was found between osteocalcin and osteoprotegerin mRNA contents and BMD values. However, patients in group A had slightly but not significantly higher level of mRNA contents of type Ⅰ collagen than that in group B. There was weak positive correlation between mRNA contents of type Ⅰ collagen and BMD values in both groups. Conclusion Although significant correlations were not found between mRNA contents of type Ⅰ collagen, osteocalcin and osteoprotegerin and BMD values, the relationship between mRNA contents of type Ⅰ collagen and osteopenia in AIS patients should not be excluded, for it could be weakened due to a larger range of patients'age.
