自然流产患者细胞因子的变化及补肾健脾中药对其干预作用的临床研究

摘　　要：目的:探讨T辅助-1型/T辅助-2型(Th1/Th2)细胞因子与自然流产的相关性及其影响因素,观察补肾健脾中药复方助孕3号方对自然流产患者Th1/Th2细胞因子的影响。方法:采用放免法检测15例正常组妊娠妇女、30例治疗组自然流产患者治疗前后血清IL(白介素)-2、IL-4、IL-10水平,ELISA法检测其血清IFN-γ(γ-干扰素)水平,比色法检测其血清NO(一氧化氮)含量。结果:与正常组相比.治疗前治疗组血清IL-10水平明显下降(P<0.05),IFN-γ/IL-10明显升高(P<0.05),Th1/Th2明显升高(P<0.05)。治疗组自然流产次数与IL-2/IL-4、IFN-γ/IL-4呈正相关(P<0.01);NO与IL-4成负相关(P<0.05).而与IL-2/IL-4、IFN-γ/IL-4均呈正相关(P<0.05)。治疗组治疗后血清IL-2、IFN-γ含量均较治疗前有所下降(P<0.05),IL-2/IL-10比值明显下降(P<0.05),Th1/Th2亦较治疗前明显下降(P<0.05)。结论:自然流产患者存在较高的Th1反应,而较高的NO的表达促进了Th1反应;助孕3号方能有效调节自然流产患者Th1/Th2细胞因子的平衡,并降低NO表达,有利于妊娠维持。Objective:To explore the correlation of cytokine Th1/Th2 with apontancous abortion (SA)and to observe the effect of herbs of tonifying kidney and strengthening spleen on cytokine Th1/Th2.Methods:Serum levels of intedeukin 2 (IL-2),IL-4 and IL- 10 in 15 healthy pregnant women and in 30 SA patients be- fore and after treatment with Zhuyun Preacription No.3 were detected with radioim- munoassay,their serum interferon gamma(IFN-γ)level was detected by ELISA and serum nitric oxide (NO)content by colorimetry.Results:Compared with the healthy pregnant women,serum level of IL - 10 decreased (P<0.05),[FN-γ/IL - 10 ra- tion increased(P<0.05),and Th1/Th2 also iecreased(P<0.05)before treat- ment.The abortion frequency was positively correlated with the ratio of IL- 2/IL- 4 and IFN-γ/IL-4(P<0.01).NO content was negative correlated with IL-4 in SA patients,while positively correlated with IL-2/IL-4 and IFN-γ/IL-4(P<0.05).IL-2 and IFN-γlevels as well as the ratios of IL-2/IL-10 and Th1/ Th2 were decreased in SA patients after treatment (P<0.05).Conclusion:There exists a high reaction of Thl type which is enhanced by high- level NO.Zhuyun Pre- scription No.3 regulates the balance of Th1/Th2,reduce the expression of NO which benefits to the maintenance of pregnancy.
