纤维乳管内镜在乳腺疾病诊治中的应用

摘　　要：目的探讨纤维乳管内镜检查对乳头溢液性乳腺疾病的诊治价值。方法应用德国Schlly公司生产的纤维乳管内镜对36例乳腺疾病患者进行诊断,直接观察乳腺导管内的病变,初步判断其疾病性质并准确定位。将检查结果与术后病理诊断进行对比分析。结果36例患者中乳管内占位性病变15例(乳管内乳头状瘤14例,乳腺导管浸润癌1例)、乳管炎11例,乳管扩张症10例(其中9例合并乳管炎)。乳头状瘤的病理检查结果与镜下诊断符合率为85·7%(12/14)。占位性病变患者经手术治疗,乳管炎和乳管扩张症患者进行镜下介入治疗。治疗后均痊愈或好转。结论纤维乳管内镜诊断乳腺疾病准确、定位可靠,是一种对乳头溢液性乳腺疾病安全有效的初步检查方法。Objective To study the value of fiberoptic galacto - endoscope in the diagnosis and treatment of breast disease with nipple discharge. Methods Using fiberoptic galacto - endoscope made by German Scholly Company, diagnosis was performed on 36 patients with nipple discharge to observe directly the lesions in mammary duct, to judge initially its peculiarity and to locate them definitely. And then the examination results were compared with the postoperative pathologic diagnosis. Results Out of the 36 mastopathy patients, 15 were found with intraductal space - occupying lesion （SOL） （ 14 cases with intracanalicular papilloma, 1 case with ductal infiltrating carcinoma ） , 11 with mastadenitis and 10 with mammary duct ectasia （MDE） （9 of them combined with mastadenitis）. The pathologic results of papillary epithelioma were coincident with galacto -endoscope diagnoses at a rate of 85. 7% （12/14）. Recovery or improvement was found in the SOL patients given surgical treatment and in the MDE patients given mirror interventional therapy . Conclusion Fiberoptic galacto - endoscope is a safe, effective and accurate method with reliable locations in the initial diagnosis of mastopathy.
