摘　　要：目的探讨宫颈癌及癌前病变(CIN)组织中P16INK4A基因表达的临床意义及P16INK4A基因检测对宫颈癌及CIN的诊断价值。方法选取2004—2006年经病理检查证实的宫颈浸润癌患者18例、CIN患者150例和慢性宫颈炎患者57例,采用EnVision免疫组化法检测组织标本中P16INK4A阳性表达情况,并进行比较分析。结果CIN组和浸润癌组患者的P16INK4A阳性表达率与慢性宫颈炎组间差异均有统计学意义(P<0·01);随宫颈病变程度加重,P16INK4A的表达强度和阳性表达率不断升高(P<0·01)。结论P16INK4A基因表达与宫颈癌及CIN病变程度及其进展有关;P16INK4A基因可作为宫颈癌和CIN早期诊断、判断病情、预测病情进展和预后的生物学指标。Objective To study the clinical significance of the expression of P16^INK4A in tissues of cervical intraepithelial neoplasia （CIN） and precancerous lesion. Methods We chose 18 patients with invasive carcinoma of cervix （ICC）, 150 with CIN and 57 with chronic cervicitis （CC） who were confirmedly diagnosed by pathologic examination from 2004 to 2005 year, and then determined the positive expression of P16 INK4A in the tissue samples by EnVision immuno - histochemical method. Results The CC group was significantly different from the ICC group and the CIN group in the positive expression of P16 INK4A （ P 〈 0. 01 ） ; with the development of cervical lesion, the expression intensity and the positive expression of P16 INK4A were elevated unceasingly （ P 〈 0. 01 ）. Conclusion The expressions of P16^INK4A, which can serve as a biological parameter for early diagnosis, judgement of pathogenetic condition, prediction of lesion development and prognosis, was related with the extent and development of CIN lesion.
