CIN患者外周血CD4、CD8 T淋巴细胞的表达及意义

摘　　要：目的探讨宫颈上皮内瘤变（CIN）不同级别的患者外周血T淋巴细胞亚群的CD4^＋、CD8^＋细胞数量的变化及与临床意义。方法采用流式细胞术分析128名CINI一Ⅲ级的患者与30例正常人静脉血CD4^＋、CD8^＋、以及CD4^＋／CD8^＋比值以及之间相关性，并对36例CIN治疗后随访。结果CINI组与正常组之间CD8值（P〉0，05）无显著差别；CINⅠ、CINⅡ与正常组CD4，CINI与CINⅡ，CINⅡ与CINⅢ的CD8差别有显著性（P〈0．05）；其他组之间都有高度显著的差异（P〈0．01）。且随CD4数量减低CD8相对增加，CD4／CD8减低；随访结果疗效显著的，CD4^＋、CD8^＋、CD4^＋／CD8^＋比值都有明显增加。疗效差的CD4^＋，CD4^＋／CD8^＋比值较治疗前减少，预后不佳。结论CD4^＋、CD8^＋细胞表达的数量随CIN患者病情变化而改变，对临床调节免疫力，提高疗效及预后的判断有重要的指导意义。Objective To explore variations of levels of T - lymphocyte subsets （ CD4^＋ , CD8^＋ and CD4^＋/CD8^＋ ） ceils and their clinical significance of cervical intraepithelial neoplasia （CIN） pat/ents. Methods The ratio and correlation of blood CD4, CD8, and CD4/CD8 in 126 CIN Ⅰ patients at 128 Ⅲ class and 30 normal controls were anlayzed by using flow cytometry. Results There was no significant statistical differences in the levels of CD8 in CIN Ⅰ patientls and normal controls were observed （P 〉 0.05）, while significant statistical differences were observed in the levels of CD4 in CINⅠ and CIN Ⅱ compared with that of normal controls, CINⅠ with CIN Ⅱ and the levels of CD8 of CIN Ⅱ patients with CIN Ⅲ patients （P 〈 0.05 ）. among other groups a highly significant statistical differences were also observed （ P 〈0.01 ）. The levels of CD4 and CD8 wwere increased relative increase, CD4/CD8 reduce their treatment poor prognosis. Conclusion Distribution of T - ceil subsets of patients with CIN change with the change of the clinical treatment of enhanced cellular immunity improve the effectiveness of mitigation have important reference value.
