纤维可视输尿管导管在盆腔粘连患者腹腔镜子宫切除术中的应用

摘　　要：目的:探讨盆腔粘连患者行腹腔镜子宫切除术时放置纤维可视输尿管导管预防输尿管损伤的可行性。方法:随机选取盆腔粘连患者177例,实验组98例于术前在膀胱镜下放置输尿管导管,对照组79例按正常程序实施手术。结果:实验组避免输尿管损伤有效率为100%,对照组有效率为91.14%,差异有统计学意义(P<0.05)。在子宫次全切除术及子宫全切除术中,实验组手术时间均明显短于对照组(P<0.001),差异有统计学意义。结论:盆腔粘连患者行腹腔镜下子宫切除术时放置纤维可视输尿管导管,可有效防止输尿管损伤,手术操作安全,快捷。Objective ： To investigate the availability of avoiding ureter injury using visible ureteral catheter in laparoscopic subtotal hysterectomy（LSH） and laparoscopic total hysterectomy（LTH）. Methods：One hundred and seventy-seven patients who suffered from pelvic pathologic adhesion were chosed, of which 98 patients used visible ureteral catheter before laparoscopic hysterectomy as study group and 79 patients underwent laparoscopic hysterectomy without visible ureteral catheter as control group. Results： For the effective rate of avoiding ureter injury in study group was 100% and that of control group was 91.14% ,the difference between the two groups was significant （P 〈 0.05 ）. The operation time of study group was shorter compared with control group in LSH and LTH （ P 〈 0.001 ）. Conclusions ： It is safe to perform operation using visible ureteral catheter in LSH and LTH, meanwhile, it can also prevent ureter injuy effectively.
