摘  要:目的:研究螺旋断层放疗系统在全中枢治疗中的剂量学优势,并对毒副反应及临床疗效做初步探讨。方法:2007年10月~2007年11月对2例髓母细胞瘤患者实施了螺旋断层放射治疗,CTV1为术后瘤床,照射剂量50.6Gy,CTV2为全脑及全脊髓,照射剂量30.6Gy,每次1.8Gy,5次/周。结果:全中枢螺旋断层放疗较常规加速器治疗简单方便、剂量分布均匀、精度高且误差小,对正常组织损伤明显减低。两例患者中,1例顺利完成治疗,出现2级血液学放射损伤;另1例患者出现3级血液放射损伤及2级眼部放射损伤症状,2例患者放疗后1月复查均未见复发。结论:断层放疗系统为全中枢照射提供了高度精确的调强放射治疗和实时的位置、剂量验证,有着常规加速器放疗所无法比拟的优势,为放射治疗医师开辟了一个新的治疗平台。Objective: To review the target coverage of helical tomotherapy (HT) plans for craniospinal radiation and its clinical resuhs and side effects. Methods: 2 patients (8-year-old and 18-year-old) with medulloblastoma were treated with tomotherapy. Prescription dose of GTV was 50.6Gy and CTV was 30.6Gy, 1.8Gy per fraction. Results: The HT plans demonstrated excellent target coverage, homogeneity and organ sparing. One of the patients completed the treatment successfully except for the haematological radiation injury (grade 1I ) ; but the other delayed two weeks during the tomotherapy because of the haematological radiation injury (grade Ⅲ ) and the eye radiation injury (grade Ⅱ ). Conclusion: The HT plans demonstrate excellent compared with a conventional linear accelerator based craniospinal radiation plan. By the basis of this case study, further evaluation of HT for craniospinal radiotherapy seems justified.
