早期肠内营养在胃底贲门癌术后的应用研究

摘　　要：目的探讨早期肠内营养应用于胃底贲门癌术后的可行性、安全性和应用价值。方法将胃底责门癌139例随机分为早期肠内营养组71例和肠外营养组68例，肠内组术后第1天起从肠内营养管输入自制营养液加少量能全力6—8d，肠外组术后第1天起经静脉输入肠外营养6～8d。分析2组患者术后平均住院日、住院费用、并发症及术后恢复情况等改变的差异。结果术后平均住院日肠内组比肠外组缩短4．9d；平均住院费用／药费，肠内组与肠外组分别为（9416，3／2799．3）元、（12943．5／4107．5）元，差异有统计学意义（P〈0．01）；并发症肠内组比肠外组减少（P〈0．01）；肛门排气、排便时间肠内组比肠外组早；血清白蛋白2组比较（J，〈0．01）。结论早期肠内营养应用于胃底贲门癌术后安全可行，既符合生理要求。促进肠功能恢复，又可改善患者术后营养状况。Objective To investigate the efficiency, safety and application of early enteral-intestinal nutrition in post-operative patients with gastric and fundus cancer. Methods 139 cases of gastric and fundus cancer patients were randomly divided into two study groups：early enteral-intestinal nutrition group （71 cases） and intra-venous nutrition group （68 eases）. Patients in early enteral-intestinal nutrition group were given home-made nutrition-liquid plus Nuriooo for 6 -8 days from the first post-operation day; intm-venous nutrition group were given venous nutrition injection for 6- 8 days from the first day of post-operation. The average time of hospitalization, spends, complications and recovery situation were recorded and compared between the two groups. Results Average residential day in the enteral-intestinal group was 4.9 days less than the intravenous group, Average spends/drugs were 9 416/2 799.3 vs 12 943.5/4 107.5, P 〈 0.01 ; the complication in the enteral-intestinal group was less than that of intra-venous group, P 〈 0.01. Time of revive evacuate function in enteral-intestinal group is earlier than that of intra-venous group, ALB in enteral-intestinal group is higher than that of intra-venous group, P 〈 0.01. Conclusion Early enteral-intestinal nutrition in pest-operative patients of gastric and fundus cancer is safe and applicable, and it can improve patients＇ post-operation nutrition situation and intestinal function.
