摘  要:目的:探讨新生儿窒息后心功能变化对脑血流动力学改变的影响。方法:窒息与正常对照组新生儿各30例,于生后24 h内分别作头颅超声检测大脑中动脉的血流动力学变化,检测指标包括收缩期峰值流速(PSFV,cm/s)、舒张期末血流速度(EDFV,cm/s)、时间平均流速(TMFV,cm/s)、阻力指数(R I)与搏动指数(PI)和超声心动图检测左心室收缩动能,检测指标包括左室射血前期/左室射血时间比值(LPEP/LVET)及左室加速时间/左室射血时间比值(LVAT/LVET)。结果:出生时窒息可引起新生儿脑血流动力学紊乱和左心室收缩功能障碍,存在心功能障碍者脑血流动力学异常更严重。结论:新生儿窒息后心功能障碍可加重脑血流动力学紊乱,应重视对窒息新生儿心脏功能的保护。Objective: To explore the influence of myocardial function on cerebral hemodynamics in asphyxiated neonates, Methods Hemodynamic parameters of middle cerebral arteries including peak systolic flow velocity (PSFV), end -diastolic flow velocity (EDFV), time -average mean velocity (TMFV), pulsatility index (PI) and resistance index (RI) in two groups were measured in 30 asphyxiated neonates and 30 healthy controls by pulse Doppler ultrasound, At the same time, the index of left ventricular systolic function include left preejection period/left ventricular ejection time (LPEP/LVET) and left ventricular acceleration time/left ventricular ejection time (LPAP/LVET) were determined in both of the two groups. Results: Birth asphyxia can lead to cerebral hemodynamic disorder and myocardial dysfunction, and the cerebral hemodynamic disorder were even more significant in neonates with myocardial dysfunction. Conclusion: Myocardial dysfunction can aggravate cerebral hemodynamic disorder in neonates with birth asphyxia.
