血浆N-末端脑钠肽前体在老年舒张性心力衰竭患者中的应用研究

摘　　要：目的对老年舒张性心力衰竭(diastolic heart failure,DHF)患者血浆N-末端脑钠肽前体(N-terminal-pro brainnatriuretic peptide,NT-proBNP)进行研究,探讨其在老年DHF中的诊断价值。方法对129例诊断为DHF的老年患者及77例正常老年人进行超声心动图检查,并抽取血样行NT-proBNP浓度检测,比较DHF患者与正常人的血浆NT-proBNP浓度以及NT-proBNP与心脏舒张功能障碍分级的关系,并作受试者工作特征(receiver operating characteristic,ROC)曲线分析。结果老年DHF患者血浆NT-proBNP浓度显著高于正常老年人(P<0.01),且与舒张功能障碍的分级显著相关(P<0.01),NT-proBNP诊断老年DHF的曲线下面积(the area under the receiver operation characteristics curve,AUC)0.765(95%CI:0.694-0.836),200pg/ml作为界值的灵敏度为82.3%,特异度为73.3%。结论老年DHF患者血浆NT-proBNP水平显著升高,且和舒张功能障碍程度相关,对老年DHF有一定的诊断价值。Objective To investigate the value of plasma NT-proBNP in diastolic heart failure（DHF） in the elderly. Methods 129 elderly DHF patients, 77 senile healthy volunteers were selected. Plasma NT-proBNP were tested using electrochemiluminescence immunoassay. Echocardiography including：LVEF,E/A,E/Vp and so on were examined. Results The level of plasma NT-proBNE increased significantly compared with controls in elderly DHF（P〈0.01 ）,NT-proBNP showed progressive increases（P〈0.01） in proportion to the diastolic dysfunction degree,when cut-off value was 200pg/ml for DHF in the elderly,sensitivity and specificity were 82.3% and 73.3%,respectively. Conclusions NT-proBNP may be a discerning marker for the detection and evaluation of DHF in elderly.
