尿胰蛋白酶原-2诊断急性胰腺炎再评价

摘　　要：目的探讨尿胰蛋白酶原-2（Trypsinogen-2,Try-2）检测对急性胰腺炎（Acutepancreatitis,AP）临床诊断价值。方法对138例急性腹痛患者同时进行尿胰蛋白酶原-2及血、尿淀粉酶检测,比较其特异性和敏感性,其中尿Try-2测定采用免疫层析法,血、尿淀粉酶采用酶学方法检测。结果138例急性腹痛患者中诊断为急性胰腺炎患者83例,尿胰蛋白酶原-2阳性者79例,敏感性95.2%（79/83）。55例为非急性胰腺炎患者,尿胰蛋白酶原-2阳性者8例,特异性85.5%（47/55）。而血淀粉酶检测的敏感性为84.3%（70/83）,特异性87.3%（48/55）;尿淀粉酶检测的敏感性为83.1%（69/83）,特异性81.8%（45/55）。尿胰蛋白酶原-2诊断急性胰腺炎的敏感性高于血、尿淀粉酶,差异有显著性（P〈0.05）,而其特异性与血、尿淀粉酶比较,差异无显著性（P〉0.05）。结论尿胰蛋白酶原-2检测对诊断急性胰腺炎有较高的敏感性,但其特异性仍值得探讨,需结合血、尿淀粉酶检测及影像检查结果综合判断。Objective To evaluate the clinical diagnostic value of urinary trypsin-2 （Trypsinogen-2, Try-2） detection ofacute pancreatitis （Acute pancreatitis AP）, Methods 138 case＇s of patients with acute abdominal pain and urinary trypsin-2 and serum amylase and urine amylase test, and compare their specificity and sensitivity, which urine Try-2 was detected by immunohistochemistry assay, serum and urine by enzyme amylase Detection methods. Results 138 cases of acute abdominal pain in patients diagnosed 83 patients with acute pancreatitis, urinary trypsin-2 positive in 79 cases, the sensitivity of 95,2% （79/83）. 55 cases of patients with non-acute pancreatitis, urinary trypsin-2 positive eight cases, specificity 85.5% （47/55）. The serum amylase test the sensitivity of 84.3% （70/83）, specificity 87.3% （48/55）： urinary amylase test the sensitivity of 83.1% （69/83）, specificity was 81.8% （45 / 55）. Urinary Trypsin-2 in the diagnosis of acute pancreatitis is more sensitive than serum and urine amylase, there were significant differences （P 〈0.05）, and its specificity and serum and urine amylase comparison, the difference was not statistically significant （P〉0. 05）. Conclusions urine trypsinogen-2 tes（ing in the diagnosis of acute pancreatitis have higher sensitivity, but its specificity is still worth exploring, with the need serum and urine amylase detection and imaging findings comprehensive judgement.
