74例腰椎间盘突出症术后中期疗效评价

摘　　要：目的评价腰椎间盘突出症术后中期疗效。方法采用电话随访结合门诊复查的形式，随访患者74例。分为椎板间开窗组、半椎板切除组、全椎板切除组、植骨内固定组。结果开窗组、半椎板组、植骨内固定组优良率分别为87．8％、75．0％和85．7％，全椎板组优良率50．0％。开窗组和半椎板组术后恢复工作时间较短，大部能从事原先工作。全椎板切除组和植骨内固定组术后恢复时间相对较长，且大部分改变工作，从事较轻工作。结论椎板间开窗术、半椎板切除术、椎板切除减压加椎体间植骨融合椎弓根钉系统内固定术术后中期疗效良好，全椎板切除术后中期疗效欠佳。Objective To evaluate the intermediate stage surgical outcomes in 74 patients with lumbar disc herniation.Methods 74 patients who underwent operative treatment for lumbar disc herniation were fol- lowed-up by the＂telephone revisit and the clinical reexamination. The patients were divided into ＂open-window＂ group,hemilaminectomy group,total laminectomy group and transpedicular screw fixation combined with interbody spinal fusion by auto bone graft group. Results The excellent rates in ＂open-window＂group, hemilaminectomy group,total lamineetomy group and transpedieular screw fixation combined with interbody spinal fusion by auto bone graft group were 87.8% ,75.0% ,50. 0% and 85.7% respectively. The time of returning to work in ＂open-window＂ and hemilaminectomy group was short and most of them can fulfill their previous work.. The time of returning to work in total laminectomy group and transpedicular screw fixation combined with interbody spinal fusion by auto bone graft group was relatively long, and most of them changed their previous hard work. Conclusion The intermediate stage surgical effect for treatment of lumbar disc herniation in ＂open-window＂, hemilaminectomy and transpedicular screw fixation combined with interbody spinal fusion by auto bone graft is satisfactory. But The intermediate stage surgical effect for treatment of lumbar disc herniation in total laminectomy is relativeiy unsatisfactory.
