摘 要:目的:探讨青霉素过敏反应产生特异性IgE抗体与人类白细胞抗原(HLA-DRB)基因多态性的相关性。方法:以河南汉族临床青霉素过敏反应的患者为研究对象,采用放射免疫吸附试验(radioallergosorbent test,RAST)检测血清中针对青霉素药物的8种特异性IgE抗体和采用序列特异性引物-聚合酶链反应(polymerase chain reaction sequence-specific primer,PCR-SSP)的方法检测HLA-DRB等位基因。结果:与对照组相比,IgE抗体阳性组DR14.1基因频率显著降低(P<0.05),并且在速发型和荨麻疹组DR14.1基因频率均明显降低(P<0.05);IgE抗体阴性组DR17、DR18、DR9、DR13.1、DR14.1基因频率均高于对照组(P<0.05)。结论:DR14.1基因与IgE抗体的产生有关,可能为抗体阳性过敏反应的保护基因,并且与速发型过敏反应和出现荨麻疹症状相关。OBJECTIVE To explore the correlation of specific IgE antibodies to penicillins allergy and HLA-DRB genetic polymorphism. METHODS Sequence specific primer-polymerase chain reaction (PCR-SSP) was used to type for HLA-DRB alleles in 113 patients with allergy to penicillins and 101 healthy subjects without any allergic reactions. RESULTS Compared with control subjects, a significantly decreased frequency of DR 14. 1 was present in 80 patients with allergic reactions (P〈0. 05), with immediate reactions and with urticaria (P〈0. 05). In the 33 patients with negative antibodies, the gene frequencies of DR17, DR18, DR9, DR 13. 1 and DR 14. 1 were notablely higher (P〈0. 05). CONCLUSION DR 14. 1 gene is associated with expression of IgE antibody, and it probably play a protective role in the process of allergic reaction in patients with positive antibodies.
