摘 要:目的:创建新的适合于中国人的多参数回归公式用于胎重估测。方法:750例孕妇,在分娩前3d以内超声检查,记录胎儿双顶径(BPD)、头围(HC)、腹围(HC)和股骨长(FL),出生后记录新生儿体重。其中600例用于公式创建,采用非线性回归设计公式,另150例用于公式的验证。结果:Log10EFW=1.0163+0.0023(HC)+0.0603(AC)+0.2571(FL)+0.0011(BPD)(AC)-0.0074(AC)(FL)(R=0.846)。在验证组,新公式与其他常用公式相比,平均绝对误差及平均绝对百分误差均为最低,准确性较高。结论:新公式更适宜于评估中国胎儿体重。Objective: To develop a new formula to estimate fetal weight for Chinese populations. Methods: As the formula finding group, a total of 750 fetuses within 3 d of delivery were considered. Twoimensional ultrasound measurements of the fetal biparietal diameter (BPD), head circumference (HC), abdominal circumference (AC) and femur length (FL) were performed and recorded by experienced ultrasound physicians. Then, nonlinear regression analysis was utilized to develop the new formula with BPD, HC, AC, and FL as independent parameters. As the formula evaluation group, another 150 fetuses' weight were estimated by the new formula and compared with other 6 well-established equations. Results: Log10EFW=1.016 3+0.0023(HC)+0.060 3(AC)+0.2571(FL)+0.001 1 (BPD)(AC)-0.0074(AC)(FL) (R=0.846) was the new formula. When tested it in the evaluation group, the new formula gave satisfactory estimation accuracy. The mean absolute error and the mean absolute percentage error were the lowest. Compared with other formula, the proposed formula yielded the best fit between the estimated fetal weight and the actual birth weight. Conclusion:The new formula is based on typical Chinese Han women, the error was lower and more suitable than those developed formulae for Western populations.
