摘 要:目的探讨Ⅱ型糖尿病胰岛素抵抗及其与C反应蛋白(CRP)、肿瘤坏死因子(TNF-α)和内皮素-1(ET-1)水平的关系,通过观察川芎嗪注射液对Ⅱ型糖尿病胰岛素抵抗的影响,初步探讨川芎嗪注射液对Ⅱ型糖尿病胰岛素抵抗的影响及机制。方法比较Ⅱ型糖尿病患者100例与正常人群30例的血清C反应蛋白(CRP)、肿瘤坏死因子(TNF-α)和内皮素-1(ET-1)水平;将Ⅱ型糖尿病患者100例随机分为中西药治疗组(A组)和常规治疗组(B组),每组各50例。2组患者分别于治疗前后测定空腹血糖、餐后2h血糖、血清C反应蛋白(CRP)、肿瘤坏死因子(TNF-α)和内皮素-1(ET-1)水平。比较2组患者治疗前后上述指标的变化。结果Ⅱ型糖尿病患者的血清C反应蛋白(CRP)、肿瘤坏死因子(TNF-α)较正常人群升高,内皮素-1(ET-1)降低(P〈0.05)。A组患者血糖控制达标时间较B组缩短,有显著性差异(P〈0.05);A组血清C反应蛋白(CRP)和肿瘤坏死因子(TNF-α)降低、而内皮素-1(ET-1)升高,较B组有显著性差异(P〈0.01)。结论Ⅱ型糖尿病胰岛素抵抗与其与C反应蛋白(CRP)、肿瘤坏死因子(TNF-α)呈正相关,与内皮素-1(ET-1)呈负相。川芎嗪注射液通过有效的改善糖尿病患者的胰岛素拮抗因子,提高患者对胰岛素的敏感性,缩短血糖控制达标时间。Objective To study the relationship between type Ⅱ diabetes mellitus's insulin resistance and C-reactive protein(CRP), tumor necrosis factor(TNF-α). endothelium-1(ET-1) level, and through observe the effect of Chuanxiongqin injection on insulin resistance ,to initial investigate the effect and mechanism about Chuanxiongqin injection on type Ⅱ diabetes mellitus's insulin resistance. Methods The first:to collect 100 patients with type 2 diabetes mellitus and 30 health subjects in our hospital. The second:100 type Ⅱ diabetes mellitus patients were randomly divided into group A(conbined treatment of traditional Chinese medicine and western medicine)and group B (conventional therapy), every group had 50 patients and to determine fasting blood sugar, postprandial 2 hour blood sugar, C-reactive protein(CRP), tumor necrosis factor(TNF-α)and endothelium-1(ET-1) levels about before and after treatment, to compare the indexes in the two groups. Resuits The C-reactive protein(CRP), tumor necrosis factor(TNF-α) were obviously higher in the diabetic patients than in the healthy control group, but endothelium-1(ET-1) obviously lower(P〈0. 05), The time of blood sugar in suitable standard in group A was superior to that in group B(P〈 0.05), C-reactive protein(CRP), tumor necrosis factor(TNF-α), endothelium-1(ET-1) in group A increased. And the improvement in group A was superior to that in group B (P〈0.01).Conclusion The type Ⅱ diabetes mellitus's insulin resistance was positively correlated with C-reactive protein(CRP), tumor necrosis factor(TNF-α),but negatively correlated with endothelium-1(ET-1). Chuanxiongqin injection has a certain effect in improving insulin rivalry factors in diabetic patients and raising insulin sensitivity, decurtating the time of blood sugar in suitable standard.
