摘  要:目的探讨冠心病患者阿司匹林抵抗(AR)的临床特征及其与C反应蛋白(CRP)、血栓素B2(TXB2)的关系。方法256例冠心病患者口服阿司匹林100 mg/d,7 d后测定血小板聚集率、CRP、TXB2;依据血小板聚集率测定结果将患者分为阿司匹林抵抗(AR)组、阿司匹林半抵抗(ASR)组和阿司匹林敏感(AS)组,比较三组间临床特征、CRP、TXB2的不同。结果冠心病AR的发生率为5.86%,ASR发生率为24.22%;与AS组相比,AR组和ASR组患者的血小板(PLT)和血浆纤维蛋白原(Fg)水平升高(P<0.05),同时ASR组患者中女性患者比例明显升高(P<0.05),AR组或ASR组患者TXB2水平明显升高(P<0.05),而CRP在三组间无明显统计学差异。结论冠心病患者存在AR现象,PLT和血浆Fg升高患者更易于发生AR,TXB2可用于检测AR现象。Objective To investigate the clinical feature of aspirin resistance (AR) and its correlation with C-reactive protein and thromboxan B2 (TXB2)in patients with coronary heart disease(CHD). Methods 256 CHD patients who have been on aspirin( 100 mg/d) for at least 7 days were recruited. Platelet aggregation function induced With adenosine diphosphate and arachidonic acid, and the correlation between AR and C-reactive protein , TXB2 levels was investigated. Results 5.86% of the patients showed AR and 24. 22% of the patients showed ASR. The fibrinogen levels and the amounts of platelet were significant higher in patients of AR or ASR than in patients of AS(P 〈0. 05). And the ratio of female was significant higher in patients of ASR than in AS( P 〈 0. 05 ). Meanwhile the levels of TXB2 was significantly higher in patients of AR or ASR than in patients of AS(P 〈 0. 05 ). There was no significantly difference with CRP in patients of AR or ASR or AS. Conclusions There is definitely AR phenomenon in patients with CAD. Patients who have higher fibrinogen levels and more amounts of platelet are more easily generate AR. TXB2 can be used as predictor of AR.
