摘 要:目的探讨凋亡调控基因Caspase-3和survivin在婴幼儿血管瘤组织中的表达及与细胞凋亡的关系。方法采用原位杂交法检测血管瘤及正常皮肤组织中Caspase-3 mRNA的表达;用免疫组化SP法检测survivin;用末端脱氧核苷酸转移酶(TdT)介导的d-UTP生物素缺口末端标记技术(TUNEL)检测凋亡细胞。结果Caspase-3mRNA阳性表达率在增生期和消退期分别48.4%和87.5%,两者相比,P<0.01,增生期与正常皮肤组织表达相比,P>0.05。survivin阳性表达率在增生期和消退期分别77.4%和45.8%,两者相比,P<0.01,消退期与正常皮肤组织相比,P>0.05。Caspase-3 mRNA和survivin蛋白表达呈负相关(r=-0.018,P<0.01)。婴幼儿血管瘤增生期和消退期凋亡指数(AI)分别为30.15±10.62和80.16±6.88。AI与Caspase-3 mRNA的表达呈正相关(r=0.405,P<0.01),AI与survivin的表达呈负相关(r=-0.362,P<0.01)。结论Caspase-3基因与凋亡抑制基因survivin参与了血管瘤中细胞凋亡的调节。Objective To investigate the regulation of gene Caspase-3 and survivin in infantile hemangiomas in different periods and significance of the expression of apoptosis, as well as the relationship with apoptosis. Methods Using in situ hybridization detection Caspase-3 mRNA expression of hemangioma and normal skin tissue. Immunohistochemical stainings for survivin was conducted on formalin-fixed, paraffin-embedded tissues of hemangiomas and normal skin tissue. The apoptotic cells in tissues of hemangioma and normal skin tissue were detected by terminal deoxynucleotide transferasemediated deoxyuridine triphosphate-biotin nick end-labeling (TUNEL). Results There was a significant difference of Caspase-3 mRNA expression between proliferative and involuting phase of hemangioma (48.4% vs 87.5% ,P 〈 0. 01 ), However,there was no significant difference of Caspase-3 mRNA expression between proliferative hemangioma and normal skin tissue( P 〉 0.05 ). Survivin protein expression rate in the proliferative and involuting phase were 77.4% and 45.8%, the difference between the two was statistically significant ( P 〈 0.01 ), there was no significant difference in the expression of involuting hemangioma and normal skin tissue ( P 〉 0.05 ). There was a significant negative correlation between Caspase- 3 mRNA and the expression of survivin. Apoptotic index (AI) in proliferative hemangiomas was lower than that in involuting phase hemangiomas ( P 〈 0. 01 ). There was a significant positive correlation between AI and the expression of Caspase- 3 mRNA ( r = 0. 405, P 〈 0.01 ), There was a significant negative correlation between AI and the expression of survivin ( r = - 0. 362, P 〈 0. 01 ). Conclusions Caspase-3 gene may play an important role in the hemangioma and it may be identified as a new therapeutic target. Inhibition of the Caspase-3's activation is the anti-apoptosis mechanism of survivin and play an important role in prognosis.
