糖尿病肾病与脂联素、超敏C-反应蛋白相关性研究

摘　　要：【目的】探讨血清脂联素、超敏C-反应蛋白(hs-CRP)在2型糖尿病肾病发生发展中的作用。【方法】测定正常人30例、2型糖尿病组(单纯糖尿病组30例、微量白蛋白尿组30例、蛋白尿组30例)患者共120例研究对象的血清脂联素、hs-CRP水平,并分析其与HbA1c、尿微量白蛋白等指标的关系。【结果】2型糖尿病(diabetesmellitus,DM)组血清脂联素水平明显高于正常对照组,而hs-CRP水平明显高于正常对照组(P<0.01)。糖尿病组中微量白蛋白尿组和蛋白尿组两个亚组血清脂联素及hs-CRP水平明显高于单纯糖尿病组(P<0.01)。hs-CRP与脂联素、HbA1c、尿微量白蛋白正相关(F=6.29,P<0.05)。【结论】脂联素、hs-CRP与糖尿病肾病发生、发展有关,检测血清二者水平可能对早期诊断和评估糖尿病肾病有作用。[Objective] To investigate the effect of adiponectin and supersensitive C reaction protein （hs-CRP） on the production and progress in the type 2 diabetic nephropathy. [Methods] Levels of adiponectin, hs-CRP in 120 subjects were measured; there were 30 cases normal control, 30 cases with T2DM, 30 cases with T2DM accompanied with microalbuminuria and 30 cases with T2DM accompanied with albuminuria . The relationship between adiponectin, hs - CRP, HbAlc, urinary microalbum and other parameters were analyzed. [Results] Levels of serum adiponectin in type 2 diabetic group were significantly higher than those in control group. Levels of hs-CRP in type 2 diabetic group were significantly higher than those in control group （ P 〈 0.01）. In diabetic group, levels of serum adiponectin and hs-CRP in microalbuminuriaand albuminuria groups were significantly higher than in patients only with diabetes （ P 〈 0.01）. There were positive correlation between hs-CRP and adiponectin, HbAlc, microalbuminuria （F = 6.29, P 〈 0.05）. [Conclusions] Adiponectin, hs-CRP are correlated with diabetic nephropathy. Levels of serum adiponectin and hs-CRP may have affect on early diagnosis and estimation of diabetic nephropathy.
