摘  要:[目的]通过跟踪监测乘客在青藏铁路列车中血氧饱和度(SaO2)、心率(HR)、血压(BP)、高原反应症状的变化,评估列车在高原行进中乘客的安全性,为列车中的高原医学研究提供科学依据。[方法]使用脉氧仪、血压计、改进的LakeLouise急性高原病(AMS)自评表对8名健康乘客进行全程跟踪监测,对13名自愿乘客进行抽查监测。[结果]乘客的SaO2与列车行使海拔显示出强或中相关性,在格尔木至拉萨段有的乘客会表现出较严重的SaO2下降(﹤70%),有的乘客会表现出BP显著增高(全程收缩压极差达46mmHg),有的乘客出现较严重的高原反应症状。[结论]本次监测结果显示,现行列车低氧防护措施不能完全防止AMS发生,故加强青藏列车中乘客的医学数据监测和列车低氧防护研究显得仍有必要;建议在列车上装配必要的小型高原反应监测仪器;可探索建立乘客SaO2与列车行使海拔关联性的预测数据模型研究。[Objective] To evaluate the safety of train passengers on the plateau road, we tracked and monitored oxygen saturation (SaO2), heart rate (HR), blood pressure (BP), and the changes of altitude sickness symptoms, to provide scien-tific evidence for plateau medical research. [ Methods] Tracked and monitored 8 healthy passengers in the whole process and checked 13 volunteers randomly by using pulse oxygen device, sphygmomanometer, improved Lake Louise acute mountain sickness (AMS) self-assessment forms. [ Results ] Passengers' oxygen saturation (SaO2) showed high or medium correlation with altitude. Some passengers showed a severe decrease of SaO2( 〈 70% ), some passengers had a significant increase of BP (the range of systolic pressures was 46mmHg) and some others had serious AMS. [Conclusion] The results show that present protective measures on hypoxia on the train can not prevent AMS. Therefore, it is necessary to strengthen the monitoring of the medical data of the passengers on the train and protective research on hypoxia. So, we suggest the train should be equipped with portable mini monitors and preventive system. At the same time, data model for forecasting the associativity of passengers' SaO2 and altitude that train reach to should be established.
