外来儿童免疫预防接种影响因素研究

摘　　要：[目的]了解学龄前外来儿童免疫预防接种现状及其影响因素,为采取相应的干预措施提供依据。[方法]采用横断面研究的流行病学方法,问卷调查了1528名外来儿童家长关于免疫预防服务的相关问题和儿童的接种情况。[结果]外来儿童建证率为90.4%,建卡率为87.4%,卡介苗、脊髓灰质炎、百白破三联、麻疹和乙肝疫苗单苗合格率分别为66.8%、72.6%、71.9%、79.9%和67.3%,"五苗"全程合格率为46.1%。Logistic回归结果显示母亲文化程度、儿童流动性、儿童是否在京出生、外来儿童家长对免疫预防知识的知晓率和态度、免疫接种服务质量对外来儿童免疫接种有显著性影响。[结论]为了改变外来儿童免疫预防接种现状,应该加强免疫预防知识的宣传,提高外来儿童家长参与免疫预防的自觉性和主动性,改善免疫预防服务工作质量。[Objective] To know the present situations and it＇s influencing factors of expanded program on immunization （EPI） service among the floating children, and to provide evidence for the intervention measures. [Methods] The method of cross-sectional survey was used in this study. We investigated 1528 floating children＇s parents on EPI service state by means of questionnaires. [Results] 90.4% of the floating children had immunization certificates and 87.4% of the floating children had immunization cards. The eligible immunization coverage rates of bacillus of calmette-guerin （BCG）, oral polio vaccine （OPV）, diphtheria pertussis tetanus vaccine （DPT）, measles vaccine （MV） and Hepatitis B vaccine （HBV） were 66.8%, 72.6%, 71.9%, 79.9% and 67.3% respectively. The eligible immunization coverage rate of five vaccines was 46.1% among the floating children. Logistic regression analysis indicated that the eligible immunization coverage rate of five vaccines among the floating children associated with maternal education level, flowability of children, children＇s birth place whether was in Beijing, their parents＇ knowledge and attitude to EPI, and EPI service＇ quality. [Conclusion] In order to improve immunization quality, it is necessary to strengthen the EPI education and propaganda, enhance service quality.
