摘  要:目的研究胺碘酮在风湿性心脏病瓣膜置换术后心房颤动(简称房颤)自动复律患者围术期的窦性心律(简称窦律)维持作用。方法84例瓣膜置换术后房颤自动复律的风湿性心脏瓣膜病患者随机分为试验组(n=44例)和对照组(n=40例)。对照组术后给予常规药物和安慰剂。试验组除常规药物外,术后加用胺碘酮。比较两组房颤复发率。试验终点为术后第30天。结果试验组房颤发生率小于对照组(P<0.05);术后30天窦律维持率试验组显著高于对照组(77.3%vs0,P<0.05)。结论胺碘酮能够有效地维持瓣膜置换术后房颤自动复律围术期的窦律,预防房颤的复发。Objective To evaluate the use of amiodarone to maintain sinus rhythm after cardiac valve replacement in rheumatic heart disease patients with self-recovery of atrial fibrillation. Methods Eighty-four rheumatic heart disease patients with self-recovery of atrial fibrillation after cardiac valve replacement were randomly divided into trial group (n = 44) and control group (n = 40). The control group was given routine medicine and placebo after the operation of cardiac valve replacement, while the trial group also took medicine as the control group ,only just amiodarone was added after the surgery. The incidence of postoperative atrial fibrillation were compared between the two groups. 30 days after operation was defined as the end point. Result The incidence of atrial fibrillation at 3,7 days after operation of cardiac valve replacement was significantly lower in trial group than that in control group ( P 〈 0.05 ). After 30 days, the maintenance of sinus rhythm in trial group was significantly higher than that in control group ( 77.3 % vs 0, P 〈 0.05 ). Conclution Amiodarone is effective to maintain sinus rhythm after cardiac valve replacement in rheumatic heart disease patients with self-recovery of atrial fibrillation and reduce recurrence rate of atrial fibrillation.
