308例宫颈病变诊断及治疗的研究

摘　　要：目的:了解阴道镜及HPV-DNA检测在指导宫颈病变诊断及治疗中的价值。方法:将308例阴道镜诊断可疑的HPV感染者同时检测宫颈分泌物的HPV-DNA并镜下活检。20例未治疗,267例在阴道镜下行宫颈环状电热切除术及21例宫颈癌患者根据期别做相应范围的手术治疗。术后3、6、12个月随访,行阴道镜检查及检测HPV-DNA。结果:308例阴道镜下可疑HPV感染者中,HPV-DNA检测阳性64例,阴道镜检出率20.78%;镜下活检病理CIN44例,原位癌20例,浸润癌7例,尖锐湿疣7例,HPV感染2例,检出率为25.97%。结论:阴道镜配合HPV-DNA检测指导宫颈环状电热切除术治疗CIN或HPV感染患者具有低残留、低复发、高治愈的特点。Objective To evaluate the value of colposcopy and HPV-DNA testing for diagnosing and treating cervical pathologic change. Methods Biopsy under colposcopy and HPV-DNA testing of cervical secretion were all performed in 308 patients with suspicious HPV infection under colposcopy. Twenty cases didn＇t receive any treatment, 267 cases were treated by loop electrosurgical excision procedure under colposcopy and 21 cases were operated according to their clinical stages. Colposcopy and HPV-DNA testing were both taken in 3, 6 and 12 months after operations. Results HPV-DNA positive cases were 64（20.78%） in 308 patients of suspicious HPV infection under colposcopy. Biopsy under colposcopy confirmed CIN in 44 cases, carcinoma in situ in 20, invasive carcinoma in 7, condyloma acuminate in 7 and HPV infection in 2. The positive rate of colposcopy was 25. 97%. Conclusion The treatment of CIN or HPV infection by loop electrosurgical excision procedure under colposcopy with HPV-DNA testing has few remains, low recurrence and high curative effect.
