几种抗精神病药物致心律失常的分析

摘　　要：目的:探讨几种抗精神病药物导致心律失常的因素。方法:对42例出院诊断有心律失常与同期出院的无心律失常的53例精神分裂症患者的资料进行回顾性分析。结果:发生心律失常的精神分裂症患者的性别、年龄、病程、精神药物使用时间与无心律失常的患者相比较无显著性差异(P(0.05),而与抗精神病药物种类剂量有显著关系(P<0.05、0.01)。心律失常的发生率依次为氯氮平38.1%、氯氮平维思通合用30.95%、奋乃静16.67%。在心律失常的发生中,以窦性心动过速(50%)发生率最高。结论:在较大剂量时,抗精神病药物氯氮平、奋乃静、氯氮平维思通合用易导致精神分裂症患者出现心律失常,维思通、阿立哌唑是相对安全的抗精神病药物。Objective：To study the factor of several antipsychotic cause to cardiac arrhythmia. Methods：Review and analyze the 42 patients leaved hospital were diagnosed as cardiac arrhythmia and the 53 schizophrenia patients did not diagnosed as cardiac arrhythmia in the same time. Results. The schizophrenia patients case to cardiac arrhythmia in sex, age, process of illness, time of use antipsychotic contrast to patients who had not cardiac arrhythmia no differences （P 〉 0.05 ） , but had extremely remarkable differences in kinds and dosage of antipsychotic（P 〈 0.05,0.01 ）. The occur of cardiac arrhythmia in turn as Clozapine 38.1% , Clozapine and Risperidone 30.95% ,Perphenazine 16.67%. In the occur of cardiac arrhythmia ,the highest in the cardiac arrhythmia as sinus tachycardia. Conclusions： At bigger dosage , the antipsychotic Clozapine ,Perphenazine, Clozapine and Risperidone cause cardiac arrhythmia easily, Risperidone ,Ariprazole as relative security antipsychotic.
