摘 要:目的观察无创正压通气(NIPPV)治疗慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭患者的临床疗效。方法COPD急性加重并发Ⅱ型呼吸衰竭的患者128例分为两组,常规治疗组64例,给予用鼻导管持续低流量吸氧、药物等常规治疗,NIPPV组64例,给予常规治疗的同时加用NIPPV治疗。监测两组治疗前后血气参数和呼吸频率变化。结果NIPPV组治疗后4h,PaO2即有显著上升(与治疗前比较P<0.05)。治疗后24h,pH值上升、PaCO2下降,与治疗前比较均有显著差异(P<0.05)。而常规治疗组治疗前后均无显著差异(P>0.05)。治疗组PaCO2和呼吸频率显著降低。结论NIPPV呼吸机治疗COPD伴Ⅱ型呼吸衰竭疗效显著。Objective To investigate the clinical effect of non-invasive positive pressure ventilation( NIPPV ) in patients with chronic obstructive pulmonary disease(COPD) with Type Ⅱ respiratory failure. Method The patients were divided into two groups at random. The conventional therapy group received low-flow oxygen therapy by nasal catheter continually. The treatment group received one more treatment by NIPPV. The two groups were monitored on the arterial blood gas parameters and breathing rate before and after the treatment. Results In the NIPPV group, there was a significant improvement of PaO2 at 4th hours after the therapy. PaO2 and PH were also improved obviously at 24th hours after the therapy compared with the conventional therapy group ( P 〈 0. 05 ). In the conventional therapy group, PaO2 and PH did not improve significantly at 4th or 24th hours after the therapy ( P 〉 0. 05 ). The PaO2 and breathing rate of the NIPPV group were significantly decreased after mechanical ventilation treatment. Conclusion The effect of Type Ⅱ respiratory failure from COPD treated by NIPPY is apparent.
