摘　　要：目的探讨血清高敏C反应蛋白(hsCRP)及肌钙蛋白I(cTnI)在心力衰竭患者中的变化及意义。方法46例(NYHA分级Ⅲ～Ⅳ级)心力衰竭患者,Ⅲ级组26例,Ⅳ级组20例,分别于清晨空腹和心功能改善至I～Ⅱ级后采血清,测定hsCRP和cTnI。选取45例体检健康者作为对照组。结果心衰组hsCRP和cTnI均较对照组升高(P<0.01);心衰组中心功能Ⅳ级组与Ⅲ级组比较cTnI值升高(P<0.01),而hsCRP无明显差异。心衰组随着心功能的改善hsCRP和cTnI明显下降,与治疗前比较差异具有统计学意义(P<0.01)。另外,不同病因心衰组hsCRP和cTnI差异均无统计学意义。结论心衰时hsCRP和cTnI均升高,且随着心功能损害加剧而增高明显,心衰患者存在明显的炎性活动和心肌细胞损害,并与病情严重程度相关。提示hsCRP和cTnI可能是反映心力衰竭病情变化的两项指标。Objective To investigate the changes and values of serum hypersensitive C reactive protein （hsCRP） and cardiac troponin I（cTnI） in patients with heart failure. Methods The levels of serum hsCRP and cTnI were measured in 46 patients with beart failure （NYHA＇S heart function class） NYHA,class Ⅲ group and class IV group were examined after admission. There was 26 cases for the class Ⅲ group and 20 cases for class IV. 45 healthy cases were elected for compared. Results The serum concentrations of hsCRP and cTnI were higher in patients than those in healthy group（P〈0.01 ）.The concentrations of cTnI was higher in patients of classIV group than those of class Ulgroup（P〈0.01 ）,but there was no difference between class IV and Ⅲ at the concentration of hsCRP.When the heart functions were improved. The difference o：f the levels of hsCRP and cTn I between the heart failure group and the improved group was obviously. Otherwise there was no correlation among heart failure groups with different etiologies. Conclusion In patients the severe degree of heart failure was matched to the Inflammatory activities and myocardial damage, the levels of hsCRP and cTnl could reflect the severity of heart failure. And it suggested that hsCRP and cTnI could index to the variations of the heart failure patients.
