BNP和Hs-CRP检测对急性心肌梗死及梗死后心力衰竭的诊断价值

摘　　要：目的:探讨B型钠尿肽(BNP)和超敏C反应蛋白(Hs-CRP)对急性心肌梗死(AMI)和AMI后心力衰竭(HF)的诊断价值。方法:110例初发AMI患者包括未发生HF患者58例和发生HF患者52例,均于入院第2天清晨空腹抽血,进行血浆BNP和血清Hs-CRP检测。结果:有无HF的两组AMI患者BNP、Hs-CRP水平均显著高于健康对照组(P<0.01);AMI后发生HF组BNP、Hs-CRP水平明显高于未发生HF组(P<0.01);BNP与Hs-CRP联合检测对诊断AMI后发生HF的敏感性为88.5%,特异性为81.0%,明显优于BNP或Hs-CRP单项检测。结论:BNP和Hs-CRP在AMI及HF患者中显著升高,两者联合检测对临床诊断和治疗AMI及HF有积极意义。Objective:To approach predictive value of the serum level of B-type natriuretic peptide(BNP)and high-sensitivity C-reactive protein(Hs-CRP)in acute myocardial infarction(AMI)and the heart failure(HF)after AMI.Method:A total of 110 patients with first AMI were included in this study.The BNP and Hs-CRP of each patient were examined by biochemistry analysator on the secondary day after in hospital.Results:The levels of BNP and Hs-CRP of AMI group were significant higher than normal control(P<0.01),the levels of BNP and Hs-CRP of HF patients were significant higher than no HF patients after acute myocardial infarction(P<0.01).The specificity of BNP and Hs-CRP together examnination for HF after acute myocardial infarction was 88.5%,the sensitivity was 81.0%.Conclusion:The levels of BNP and Hs-CRP are very valuable for the diagnosis and cure of AMI and HF.
