摘　　要：目的探讨实时三平面超声心动图组织多普勒评价冠心病患者左室内非同步运动的临床价值。方法对30例冠心病患者和28例正常人,应用实时三平面超声心动图技术,采集心尖四腔观三平面组织速度图像,取左室6个壁的基底段和中间段,总共12个节段,测量各节段的收缩达峰时间(Ts),计算最大差值(Ts-maxD)和标准差(Ts-sD)。测量各节段的舒张早期达峰时间(Te),计算最大差值(Te-maxD).结果冠心病组的Ts-sD(43.7±11.5)ms、Ts-maxD(137.9±38.2)ms和Te-maxD(121.3±34.7)ms明显大于正常组Ts-sD(26.8±7.4)ms、Ts-maxD(62.7±17.3)ms和Te-maxD(59.3±15.6)ms,P<0.01。结论冠心病患者左室内存在收缩舒张不同步现象,实时三平面组织多普勒成像技术为准确评价不同步运动提供了新方法。Objective To evaluate the left intraventricular systolic and diastolic asynchrony in patients with coronary artery disease （CAD） on real-time 3-plane echocardiography （RT-3PE） and quantitative tissue velocity imaging （QTVI）.Methods Thirty patients with CAD （case group ） and thirty healthy subjects （control group） underwent real-time 3-plane echocardiography. The tissue velocity images at apical four-chamber view were obtained. The 6 walls of the left ventricle were divided into basal and middle segments （12 segments） . Time to peak systolic velocities （Ts） was measured and the standard deviation of Ts （Ts-sD） and the maximal difference in Ts （Ts-maxD） between any two segments were calculated. Time to peak early diastolic velocities （Te）was measured and the maximal difference in Te （Te-maxD） was calculated. Results The Ts-sD, Ts-maxD and Te-maxD in patients with CAD （43.7±11.5 ms, 137.9±38.2 ms and 121.3±34.7 ms） were longer than those in control subjects （vs 26.8±7.4 ms, 62.7±17.3 ms and 59.3±15.6 ms respectively, P〈0.01） .Conclusions Left intraventricular systolic and diastolic asynchrony is detected in patients with CAD. RT-3PE and QTVI are new and accurate methods in assessment of the left intraventricular asynchrony.
