对2型糖尿病早期患者颈动脉弹性的临床研究

摘　　要：目的血管回声跟踪(ET)技术评价2型糖尿病早期患者颈动脉弹性参数的改变。方法选取病程在1年以内的2型糖尿病早期患者37例,正常对照组31例,ET技术检测两组颈总动脉弹性参数,并分析其与空腹血糖(FBG)、糖化血红蛋白(HbA1c)、空腹胰岛素水平(FINS)之间的关系。结果糖尿病组颈动脉弹性系数(Ep)和僵硬度(β)较对照组明显升高(P<0.05),颈动脉的顺应性(AC)两组间差异无显著性(P<0.05)。FBG和HbA1c与Ep和β呈显著正相关,与AC之间存在显著负相关;FINS与这些弹性参数相关性不显著。结论2型糖尿病早期患者颈总动脉弹性降低,其程度与FBGHbAlc明显相关。Objective To evaluate the elasticity of the carotid artery in patients with initially diagnosed type 2 diabetes mellitus （DM） using echo-tracking technique. Methods Thirty-seven patients of type 2 DM initially diagnosed less than one year were enrolled and the control group of 31 healthy volunteers was included. The carotid artery elasticity of each group was evaluated using echo-tracking technique and the possible influencing variables such as fasting blood glucose （FBG）, hemoglobin Alc （HbAlc） and fasting serum level of insulin （FINS） were measured and analyzed. Results Pressure-strain elastic modulus （Ep） and stiffness index （β） of the carotid artery were higher in type 2 DM group than those in the control group （P（0.05）, but arterial compliance （AC） showed no significant change （P〉0.05）. HbAlc and FBG showed significant correlation with Ep and [3 positively and with AC negatively while FINS showed no correlation with these parameters. Conclusions The elasticity of the carotid artery in initially diagnosed type 2 DM eases is decreased which has significant correlation with HbAle and FBG.
