经直肠超声引导下前列腺穿刺治疗前列腺脓肿的研究

摘　　要：目的探讨经直肠超声（transrectal ultrasonography，TRUS）引导下前列腺穿刺甲硝唑冲洗并抗生素保留治疗前列腺脓肿的效果。方法对21例前列腺脓肿患者在TRUS引导下穿刺前列腺至脓腔内，抽吸脓液后再用甲硝唑反复冲洗，直至清亮为止，再注入丁胺卡那霉素0．4g保留。结果21例前列腺脓肿患者1次治愈9例，2次治愈8例，3次治愈4例，穿刺成功率100％，脓液细菌培养需氧革兰氏阴性杆菌14例，金黄色葡萄球菌7例，药敏实验证实对甲硝唑、丁胺卡那霉素敏感。患者穿刺治疗后临床症状减轻或消失，患者无不良反应及并发症。经3～6个月复查随访，无1例复发。结论TRUS穿刺治疗前列腺脓肿对患者损伤小，痛苦轻，治愈时间短，是一种理想的治疗方法。Objective To explore the efficacy of transrectal ultrasound （TRUS） guided prostatic puncture and antibiotic retention for prostatic abscess treatment. Methods Prostatic abscess puncture drainage, metronidizole wash and amikacin 0.4 g retention were applied to 21 patients. Results Prostatic abscess drainage was successfully applied in 21 patients in whom 9 cases was cured at the first procedure; 8 at second; 4 at third. The outcome of the bacterial culture and medicine sensitive test showed： Gram negative bacilli were in 14 cases ; Staphylococcus aureus in 7; all were sensitive to metronidizole and amlkacin. After the drainage, all clinical symptoms disappereed, and no recurrence was in 3 miniinvasive and easy to 6 months follow-up. Conclusions The TRUS guided prostatic puncture is painless, perform and is a reasonable therapy for prostatic abscess.
