摘 要:目的:观察氟伐他汀联合地塞米松对氟伐他汀(BLM)引起的大鼠肺纤维化的治疗作用,探讨肺纤维化治疗的新方法。方法:SD大鼠随机分为5组:C组(生理盐水对照组)、BLM组(肺纤维化组)、Flu组(BLM+氟伐他汀),Dex组(BLM+地塞米松)及联合治疗组(BLM+氟伐他汀+地塞米松)。各组动物于气管内灌注BLM后第13、7、1、42、8 d分别随机处死5只。取肺组织,HE染色,作病理学观察;肺组织匀浆检测羟脯氨酸(HYP)变化;支气管肺泡灌液(BALF)测定巨噬细胞、中性粒细胞及淋巴细胞在总细胞中的百分比;上清液中ELISA法检测转化生长因子-β(TGF-β)及单核细胞趋化蛋白-1(MCP-1)变化。结果:氟伐他汀及地塞米松均可有效减轻肺组织炎症及纤维增生,减轻肺组织HYP含量,减少BALF中细胞总数及中性粒细胞、淋巴细胞百分比,降低其中TGF-β及MCP-1的含量。二者联合应用则降低肺组织HYP、BALF中TGF-β、MCP-1更早/或更显著。结论:氟伐他汀及地塞米松对大鼠实验性肺纤维化均有一定治疗作用,而二者联合应用更为有效。To investigate the therapeutic effects of fluvastatin(Flu) combined with dexamethasone(Dex) on bleomycin(BLM) - induced pulmonary fibrosis in rats. Methods:SD rats were divided randomly into five groups, i.e. group C(saline controlled), group BLM(BLM intratracheal instilled), group Flu(treated with BLM intratracheal instilled and fluvastatin), group Dex(treated with BLM intratracheal instilled and Dex) and group combined group(BLM intratracheal instilled and treated with both Flu and Dex). Five rats in each group were sacrificed in 1, 3, 7, 14 and 28 d after BLM instillation. Histological changes of their lungs were evaluated. The eontent of eollagen in the lung tissue was assessed by hydroxyproline(HYP) eoncentration. Alveolar macrophage, polyrmorphonuclear leukocyte and lymphocyte in broncboalveolar lavage fluid(BALF) were eounted. Transforming growth factor - β(TGF- β) and nxmocyte chemoattractant protein - I(MCP-1) in BALF were detected by ELISA. Results: The extent of alveolitis and pulmonary fibrosis were improved by Flu and Dex as compared with that of group BLM. The HYP concentration of group Flu and group Dex were significantly lower than that of group BLM after 7 d when the rats were administered with BLM. Total cell counts and percentage of polymorphonuclear leukocyte and lymphocyte in BALF were significantly reduced in group Flu and group Dex. TGF- β and MCP - 1 in BALF were also lower in group Flu and group Dex than those in group BLM. Moreover, combination of Flu and Dex eould reduce the content of HYP in the lung tissue and TGF- β and MCP - 1 in BALF earlier and/or more remarkably. Conclusion:Both Flu and Dex eould alleviate BLM - induced pulmonary fibrosis in rats and their combination would have a better therapeutic effect.
