131例孕产妇死亡原因分析与干预措施

摘　　要：目的探讨吉安市孕产妇死亡相关因素及降低孕产妇死亡的有效干预措施。方法回顾性对2001~2006年吉安市131例死亡孕产妇进行死因及相关因素分析。结果吉安市2001~2006年孕产妇死亡总体呈下降趋势,由2001年54.71/10万降至2006年44.08/10万,下降幅度19.43%（P〉0.05）。孕产妇死亡原因前4位为：产科出血、妊娠合并内科疾病、羊水栓塞、妊娠期高血压疾病。131例死亡孕产妇评审结果：可避免死亡占51.9%,创造条件可避免死亡占26.0%,不可避免死亡占22.1%;死亡地点县级以上医院占55.0%,乡级医院占22.1%,家中占8.4%,途中13.0%,其它2.3%。结论吉安市孕产妇死亡率仍有下降空间。加强各级医院产科建设,特别是乡镇卫生院的产科建设,落实孕产妇系统保健管理措施能有效降低本辖区内孕产妇死亡率。Objective To investigate the causes of pregnant and parturient women＇s death and the effective intervention measurement to reduce the death of pregnant and parturient women in Ji＇an city. Methods The death causes and relevant factor of 131 dead pregnant and parturient women＇s in Ji＇an city from 2001 to 2006 were analyzed retrospectively. Results The trend of change of the mortality of pregnant and parturient was declined in Ji＇an city from 2001 to 2006： from 54.71/10^5 in 2001 to 44.08/10^5 in 2006,and the decline extend was 19.43%（P〉0.05）. The four principal causes of death were obstetric hemorrhage,internal medicine disease,amniotic fluid embolism and pregnancy induced hypertension. The results of estimate and diagnose of the 131 dead pregnant and parturient women were： voidable death 51.9 % ,voidable death of creating con- ditions 26.0%,invoidable death 22.1%. The places of death were：55.0% in urban hospitals,22. 1% in rural hospitals,8. 4% in houses,13.0% on the way and others 2.3%. Conclusion The death rate of pregnant and parturient women had still reducing space. Reinforcing the obstetrical department＇s construction of all of the various levels hospitals and women＇s system health care could reduce effectively the death rate of pregnant and parturient women in area under self＇s jurisdiction.
