牙科高速手机和印模HBV污染和消毒效果的比较观察

摘　　要：目的了解临床牙科高速手机和印模的乙型肝炎病毒(HBV)污染状况,评价几种不同方法对牙科高速手机和印模的HBV消毒效果。方法对口内操作30 min后的高速手机、硅橡胶印模、藻酸盐印模各1 000件进行HBsAg抗原性定量检测。将检测结果为阳性的样本随机分成两组,其中高速手机分别采用蒸汽压力灭菌和紫外线消毒器消毒,对印模分别采取健之素消毒液浸泡和紫外线消毒;再次进行HBsAg抗原性定量检测并进行比较。结果(1)HBV污染状况:手机和印模的HBsAg污染阳性率为10%～14%。(2)手机消毒:压力蒸汽法HBsAg阳性率为0,紫外线消毒法为47%,两种消毒法组间差异有统计学意义(P<0.01)。(3)硅橡胶与藻酸盐印模消毒:健之素消毒液浸泡后HBsAg阳性率为0,紫外线消毒后阳性率分别为18%、28%,两种消毒法组间差异有统计学意义(P<0.01),但紫外线消毒法对两种印模的消毒效果差异无统计学意义(P>0.05)。结论口内操作后手机和印模污染严重,紫外线消毒的效果并不理想,压力蒸汽法和健之素消毒液浸泡是可靠的消毒方法。Objective To investigate the HBV contamination of dental hand piece and impression and to compare the effect of different disinfection methods. Methods 1 000 dental hand pieces and silastic and ALG impressions 30 minutes after oral use were tested with Quantitative HBsAg antigen. The positive samples were randomized into two groups. The hand pieces were disinfected with vapor pressure and ultraviolet light and the impressions with soaking in sterilizing agents and ultroviolet light, followed by tests with HBsAg antigen. The sterilization effects were compared. Results （1） The HBV positive rate for hand pieces and impressions was 10%-14%.（2）As for sterilization of hand pieces, the positive rates for vapor pressure and for ultraviolet light were zero and 47%, respectively, with significant difference （P〈0.01）. （3） As for sterilization of impressions, the positive rate for sterilizing agents and ultraviolet light were zero and 18%, 28%, with significant difference （P〈0.01）. But there was no significant difference for ultraviolet light to sterilize impressions （P〉0.05）. Conclusion The HBV contamination of dental hand pieces and impressions is serious. Their disinfection with vapor pressure and disinfecting agents is safe and effective hut ultraviolet light is unsatisfactory for sterilization.
