摘  要:目的研究伴有高危型人乳头瘤病毒(high-risk human papillomavirus,hr-HPV)感染及无hr-HPV感染的宫颈癌(cervical cancer,CC)、宫颈上皮内瘤变(cervical intraepithelial neoplasia,CIN)及宫颈炎中p16INK4a蛋白表达水平的变化及意义,探讨宫颈癌及癌前病变中p16INK4a蛋白表达与HPV感染的相关性。方法应用免疫组化方法检测126例宫颈疾病(CC40例,CIN67例和宫颈炎19例)组织中p16INK4a蛋白的表达,其中,hr-HPV(+)宫颈疾病89例(CC32例,CIN48例和宫颈炎9例),hr-HPV(-)宫颈疾病37例(CC8例,CIN19例和宫颈炎10例),进行半定量分析及统计学检验。结果40例CC中,p16INK4a全部阳性表达;48例hr-HPV(+)和19例hr-HPV(-)CIN中,p16INK4a阳性表达分别为39例和12例;9例hr-HPV(+)和10例hr-HPV(-)宫颈炎组织中,p16INK4a阳性表达均为4例。hr-HPV(+)或hr-HPV(-)宫颈炎、宫颈上皮内瘤变和宫颈癌组织中p16INK4a蛋白表达水平均依次升高。CC和CIN中p16INK4a蛋白表达与hr-HPV感染相互间无显著相关性(P>0.05)。结论细胞周期抑制蛋白p16INK4a在CC及癌前病变中过表达,且与hr-HPV感染无关,提示p16INK4a蛋白在宫颈癌的发生、发展中所起的作用和作用机制可能与其在其他恶性肿瘤中不同。检测p16INK4a蛋白表达有助于CC的早期诊断。Objective To study whether p16^INK4a protein expressions in cervical cancer (CC), cervical in- traepithelial neoplasia (CIN) and cervicitis are associated with high-risk human papillomavirus (hr-HPV) infection. Methods The expressions of p16^INK4a protein were detected in 126 samples of cervical lesions (40 CC, 67 CIN, 19 cervicitis) by immunohistoehemical staining, among which 89 cervical lesions were hr-HPV( + ) (32 CC, 48 CIN, 9 cervicitis) and 37 hr-HPV(-) (8 CC, 19 CIN, 10 eervicitis). Results p16^INK4a protein was positive in all of CC samples. The positive expression of p16^INK4a was in 39 hr-HPV ( + ) samples and 12 hr-HPV(-) CIN samples, with no significant difference between each other. The positive expression of hr-HPV infection. Pearson correlation analysis revealed no correlation between hr-HPV infection and p16^INK4a protein expressions in CC and CIN ( P 〉 0. 05). Gonclusion Overexpression of INK4a gene indicates the potential of CC development. Moreover, there is no correlation between the overexpression of p16^INK4a protein and hr-HPV state in cervical lesions. Therefore, it is likely that p16^INK4a protein expression is not induced bv the same carcinogenic mechanisms of CC and other malignant tumors.Overexpression of NIK4a gene helps in the diagnosis of cervical cancer.
