摘 要:目的探讨肉毒杆菌毒素A(botulinum toxin A,BTXA)水凝胶对大鼠骨骼肌(腓肠肌)远期作用的形态学影响及其意义。方法将BTXA行示踪标记配制125I-BTXA水凝胶与125I-BTXA水溶液,进行样品放射性强度测定。12只SD大鼠用随机数字表法分为A、B两组,每组6只,大鼠右侧腓肠肌注射BTXA水凝胶,左侧腓肠肌注射等体积生理盐水作对照,于注射后6个月(A组)、12个月(B组)切取腓肠肌标本,常规病理HE染色、1%氯化金染色,透射电子显微镜观察组织结构变化。结果125I-BTXA水凝胶与125I-BTXA水溶液放射性强度比较无统计学差异(P>0.05);光镜下大鼠腓肠肌组织形态无明显改变;透射电镜观察,注射后6个月,出现不可逆性肌纤维溶解,肌膜下线粒体嵴肿胀、空泡变,局部肌浆网增厚、扩张;注射后12个月,肌细胞损伤无明显改善。结论BTXA在水凝胶中的浓度与在生理盐水中相当,BTXA水凝胶作用的骨骼肌远期超微结构形态不能恢复正常,其对骨骼肌在远期仍有影响。Objective To investigate the long-term morphologic change SD rats after botulinum toxin A (BTXA) gel induced paralysis. Methods s of the gastrocnemius muscles of BTXA labeled with 12sI was obtained. The radioactivity measurement was taken to measure the solubility of BTXA in gel and BTXA solution (normal saline as solvent). The right gastrocnemius muscles of 12 SD rats were injected with BTXA gel and the left gastrocnemius muscles injected with normal saline were used as controls. HE staining and 1% gold chloride staining were used to study the tissue sion electron microscopy was used to changes 6 months and 12 months after BTXA gel injection and transmission electron microscopy was used to observe the ultrastructural changes. Results The radioactivity of ^125 I-BTXA gel and ^125 I-BTXA solution had no significant differences (P 〉 0. 05 ). Histopathologic examination showed no changes in the right gastrocnemius muscles injected with BTXA gel, but uhrastructurally the myopathic changes were clearly visible, like diffuse sarcomere disruption and saroplasmic reticulum expanding. The myofibre degeneration showed no remission 12 months after BTXA gel injection. Conclusion BTXA is dissolved in gel evenly. The long-lasting myofibre degeneration in BTXA gel paralyzed muscles may reflect that the paralyzed muscles fail to regain their unique function and recovery of muscle contraction.
