体外受精-胚胎移植助孕妊娠后流产的相关因素分析

摘　　要：目的探讨体外受精-胚胎移植(IVF-ET)助孕妊娠后发生自然流产的相关因素。方法对2001年1月至2005年12月在本中心行常规IVF-ET的397个周期进行回顾性分析,按不同妊娠结局进行分组,比较各组的年龄、不孕年限、启动剂量、Gn支数、Gn天数、获卵数、孕囊个数。结果未妊娠组(P=0.001)和早期流产组(P=0.037)的年龄高于早产/足月产组;未妊娠组的不孕年限高于早产/足月产组(P=0.002);未妊娠组(P=0.001)和早期流产组(P=0.021)的启动剂量均高于早产/足月产组。不同组的Gn支数、Gn天数之间差异无统计学意义。未妊娠组的获卵数低于早产/足月产组(P=0.045),晚期流产组的孕囊个数高于早期流产组(P=0.000)、宫外孕组(P=0.002)和早产/足月产组(P=0.046)。结论女性年龄、不孕年限是IVF-ET助孕妊娠后发生早期自然流产的相关因素,而多胎妊娠是晚期自然流产的高危因素。Objective To investigate the correlation factors of spontaneous abortion in assisted reproductive technology. Methods 397 cycles of patients between January 2001 and December 2005 were reviewed. Female age, duration of infertility,dosage of start ,ampoules of Gn,days of Gn,oocytes retrieved and numbers of sac of the 397 IVF cycles were analysed among the groups of the different outcomes. Results Female age of the non-pregnant group and the early spontaneous abortion group were higher than that of the premature delivery/term birth group. Duration of infertility of the non-pregnant group was higher than that of the premature delivery/term birth group. Dosage of start of the non-pregnant group and the early spontaneous abortion group were higher than that of the premature delivery/term birth group. Ampoules of Gn, days of Gn were not significantly different among the groups of the different outcomes. Oocytes retrieved of the non-pregnant group were lower than that of the premature delivery/term birth group. The number of sac of the late spontaneous abortion group was higher than those of the early spontaneous abortion group, the ectopic pregnancy group and the premature delivery/term birth group. Conclusion Female age, duration of infertility were related with early spontaneous abortion. Multiple pregnancy is a risk factor of late spontaneous abortion.
