摘　　要：目的制备Ｓａ抗原，建立抗Ｓａ抗体的检测方法，测定自身免疫性结缔组织疾病中抗Ｓａ抗体的阳性率。方法从人胎盘中提取Ｓａ抗原，用阴离子交换法进一步纯化，采用免疫印迹法，检测抗Ｓａ抗体。结果抗Ｓａ抗体与类风湿因子、抗ＲＡ３３、抗Ｒｏ（ＳＳＡ）、抗Ｌａ（ＳＳｂ）、抗ＲＮＰ、抗Ｓｍ、抗Ｊｏ－１和抗Ｓｃｌ－７０抗体等自身抗体无交叉反应。抗Ｓａ抗体在各组患者中的阳性率分别为：类风湿性关节炎（ＲＡ）３１．９％（６１／１９１），干燥综合征３．０％（２／６７），系统性红斑狼疮４．３％（２／４６），而白塞病（６６例）、多发性肌炎／皮肌炎（６０例）、其他自身免疫性结缔组织病（６６例）与正常人（４０例）中均为０。抗Ｓａ抗体对ＲＡ的特异性为９８．６％。Objective To extract Sa antigen and screen antiSaantibody in autoimmune connective tissue disease(CTD).Methods By using ion exchange method, we extracted Sa antigen from human placenta. AntiSaantibodies were tested in 40 normal controls and 478 CTD patients by Western blotting.Results Sa antigen was a protein with molecular weight of 50 000 and 55 000. There were no cross reactions between Sa antigen and other autoantibodies, i.e., antiSSA, antiSSB, antiSm, antiRNP, antiRA33, antiJo1 or antiScl70. AntiSa antibodies were positive in 31.9%(61/191) rheumatoid arthritis(RA), 3.0%(2/67) Sjgren's syndrome,4.3%(2/46) system lupus erythmatosus,0%(0/66) Behcet's disease,0%(0/60) PM/DM,0%(0/66) other CTD and 0%(0/40) normal controls. AntiSa antibody′s specificity for RA was 98.6%. Conclusion AntiSa antibody was different from other autoantibodies found in RA.
