食管癌弓上及弓下吻合术后早期血氧饱和度变化临床分析

摘　　要：目的:探讨食管癌弓上及弓下吻合术后早期血氧饱和度的变化情况,以指导食管癌患者不同术式的肺部护理。方法:选取行食管癌切除术患者87例,将行弓上吻合术患者51例作为弓上组,行弓下吻合术患者36例作为弓下组。分别于术后4h、8h、12h、24h、32h、36h、48h、72h记录吸氧和停氧15m in时的血氧饱和度。比较两组术后早期血氧饱和度的变化。结果:在吸氧条件下,弓上组术后8h、12h血氧饱和度较弓下组低(P<0.05);停氧各时段弓上组血氧饱和度均低于弓下组(P<0.05)。结论:对食管癌弓上吻合术后患者早期适当加大吸氧流量,加强肺部护理,可提高手术成功率,减少肺部并发症的发生。Objective： To study the early changes of blood oxygen saturation after the operation on esophageal cancer by esophagogastrostomy over and below aortic arch in order to give patients instruction on nursing care of lungs. Methods： Eighty seven patients who would have esophagogastrostomy were divided into esophagogastrostomy group over aortic arch （group A, n =51 ） and below aortic arch （group B, n = 36）, The blood oxygen saturation when oxygen was supplied and stopped were recorded in the different time of4h, 8h, 12h, 24h, 32h, 36h, 48h and 72h after operation and the changes of blood oxygen saturation was compared between the two groups. Results ： The blood oxygen saturation was lower, with oxygen supplied, in 8h and 12h after operation in group A than that in group B （ P 〈0. 05 ） ; The blood oxygen saturation was all lower, with oxygen stopped, at any time in group A than that in group B （P 〈 0. 05 ）, Conclusions ：The successful rate of operation can be increased and complications of lungs can be decreased by early supplying more volume of oxygen and careful nursing care of lungs for the patients with esophagogastrostomy over aortic arch.
