前列腺中央腺体病变的MRS半定量分析

摘　　要：目的探讨MRS半定量分析对前列腺中央腺体病变的定性诊断价值。方法对18例行经尿道前列腺切除术TURP患者的MRS谱线作回顾性分析,根据病理结果,将患者分为三组:以腺体为主型增生(GBPH)组7例、基质为主型增生(SBPH)组6例、中央腺体偶发癌(IDPC)组5例。比较各组患者MRS可用体素(胆碱+肌酸)/枸橼酸盐[(Cho+Cre)/Cit,CC/C]的比值及其与体素所在部位的关系。结果①中央腺体偶发癌组的CC/C平均值(1.32±0.42)大于GBPH组(0.88±0.28)和SBPH组(0.81±0.26),差异具有显著性(P<0.05);GBPH组与SBPH组比较,差异不具有显著性(P>0.05)。②组内比较,各部位CC/C平均值差异均不具有显著性(P>0.05)。组间比较,中央腺体偶发癌组与GB-PH组、SBPH组,在前列腺中央腺体的尖部、前部、中部和右侧,差异具有显著性(P<0.05);在前列腺体部、左侧、基底部、后部和尿道周围,差异不具有显著性(P>0.05)。结论MRS半定量分析可能会对前列腺中央腺体偶发癌和良性增生的鉴别诊断具有一定帮助。Objective To assess the value of （[Cho＋Cre]/Cit） in MRS in differential diagnosis of the lesions in central gland of the prostate. Methods The clinical and imaging data of 18 patients who had underwent TURP for BPH were analysed retrospectively. Three groups were divided according to the pathological findings： glandular-BPH （GBPH） 7 cases, stromal-BPH （SBPH） 6 cases and incidentally detected prostate carcinoma （IDPC） 5 cases. The CC/C value of different types of pathology was compared, and the relationship with the location of the voxels was considered. Results The mean CC/C in IDPC group （1.32±0. 42） was statistically higher than that in GBPH （0. 88±0. 28） and SBPH （0.81±0. 26） groups. There was no significant difference between the group of GBPH and SBPH. The mean CC/C in different locations had no significant difference in all three groups. The mean CC/C in apex, anterior, middle, and right side of the prostate in IDPC group was statistically higher than that in GBPH and SBPH group. The difference of CC/C between groups in the other regions had no statistically significant （P〉0. 05）. Conclusion The quantitative analysis of MRS may have some benefits in differential diagnosis of the lesions in central gland of prostate.
