复方丹皮酚滴丸对不稳定性心绞痛患者血浆炎症介质水平的影响

摘　　要：目的观察复方丹皮酚滴丸对不稳定心绞痛患者的临床疗效和对血浆炎症介质C-反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、单核细胞趋化蛋白-1(MCP-1)水平的影响。方法不稳定心绞痛患者90例,按信封抽取法随机分为常规治疗组(常规组)、复方丹皮酚滴丸组(丹皮酚组)和通心络组,每组各30例。3组均给予相应药物治疗2周。观察患者治疗后心绞痛症状疗效、心电图改善率,同时测定3组患者治疗前后血浆CRP、IL-6、TNF-α及MCP-1水平。结果丹皮酚组患者心绞痛缓解的总有效率为93.3%,显著高于通心络组(76.7%)和常规组(73.3%)(P<0.05,P<0.01),3组间心电图的改善率比较差异无统计学意义(P>0.05)。丹皮酚组治疗后血浆总胆固醇水平及炎症介质水平较治疗前明显降低(均P<0.05),且低于常规组和通心络组(均P<0.05)。常规组和通心络组比较差异无统计学意义(P>0.05)。结论复方丹皮酚滴丸在缓解不稳定心绞痛症状、降低炎症介质水平等方面疗效优于通心络胶囊及西医常规治疗。Objective To evaluate the clinical therapeutic effect of Compound Paeonol Dripping Pill （CPDP） and its effect on the levels of plasma inflammatory mediators, including C-reactive protein （CRP）, interleukin-6 （IL-6）, tumor necrosis factor-α （TNF-α） and monocyte chemotactic protein-1 （MCP-1）. Methods Ninety patients with unstable angina were randomized by enveloping method into 3 groups equally, the conventional Western therapy group （A）, the CPDP group （B）, and the Tongxinluo group （C）. The improvement of angina pectoris symptoms and electrocardiogram （ECG） was observed after 2 weeks of treatment and the levels of plasma CRP, IL- 6, TNF-α and MCP-1 were measured before and after treatment. Results The total effective rate in improving angina pectoris was 93.3% in Group B, significantly higher than that in Group A （73.3% , P 〈0.01 ） and Group C （76. 7%, P 〈 0.05 ） , while no significant difference of ECG improvement rate was found between the three groups （P 〉 0.05 ）. Plasma total cholesterol and inflammation indexes were significantly lowered after treatment in Group B （ P 〈 0.05 ）, showing a significant difference to those in the other two groups （ P 〈 0. 05 ）, but the indexes were unchanged in the other two groups （ P 〉 0.05 ）. Conclusion Effect of CPDP is better in relieving symptoms, depressing inflammatory reaction for treatment of unstable angina patients than that of Tonxinluo Capsule and conventional Western treatment.
