微创橇拨复位加空心拉力螺钉内固定治疗跟骨骨折33例

摘　　要：目的探讨微创斯氏针橇拨复位加空心拉力螺钉治疗跟骨骨折的疗效。方法应用斯氏针橇拨复位加空心拉力螺钉内固定治疗跟骨骨折33例(39侧)。按Sanders骨折分型,Ⅱ型23侧,Ⅲ型13侧,Ⅳ型3侧。结果33例均获得随访,平均随访时间16个月。按美国骨科协会之足踝临床评分系统评价术后功能:优28侧(71.8%),良6侧(15.4%),可3侧(7.7%),差2侧(5.1%)。优良率87.2%。结论采用微创斯氏针橇拨复位加空心拉力螺钉治疗跟骨骨折具有固定牢靠,并发症少,满意率高等优点,是一种治疗跟骨骨折的有效方法。Objective To evaluate the curative effect of poking reduction for calcaneus fracture with minimally invasive method and internal fixation with cannulated screws. Methods According to Sanders＇classifY.cation,33 patients （39 feet） with calcaneus fracture,including 23 of type Ⅱ , 13 of type Ⅲ and 3 of type Ⅳ ,were treated by poking reduction with Steinmann＇s pin through minimally invasive method and internal fixation with cannulated screws. Results Follwing up all the cases for 8--27 monthes aver- age 16 monthes. The post-operative function in 39 cases was evaluated according to American Orthopaedic Foot and Ankle Society ankle score system, the results were： excellent in 28 cases （71.8%）;good in 6 cases（15.4%）t fair in 3 cases（7.7%）;poor in 2 case （5.1%）. The excellent and good rate was 87. 2 %. Conclusion Because of the advantages of rigid fixation, fewer operative complications and high rate of excellent results,treatment of calcaneus fracture by poking reduction with Steinmann＇s pin through minimally invasive method and internal fixation with cannulated screws is an effective method.
