以性病诊疗为依托的高危男性预防艾滋病干预项目效果分析

摘　　要：[目的]探索以性病门诊为依托的男性高危人群预防艾滋病干预模式。[方法]采用追踪研究设计。以云南省某省级医院皮肤性病科门诊为依托,对在该门诊寻求性病诊疗服务且有过寻求性服务经历的男性患者提供3个月的临床诊疗与行为干预相结合的服务。干预前后进行行为监测问卷调查。[结果]干预后,男性高危人群对艾滋病传播途径知识和易混知识的正确认识率分别提高了9.2%和50%;与固定性伴发生性行为时经常使用安全套的比例由56.1%提高到了70.7%,而与非固定性伴及女性性工作者发生性行为时经常使用安全套的比例分别提高了2.1%和15.5%。[结论]临床诊疗与行为干预相结合的干预模式对男性高危人群艾滋病知识提高是行之有效的,但今后应将艾滋病的非传播途径和预防性措施知识普及当作重点。其行为也朝着较好的方向发生变化。患者退出是临床研究面临的一个重大挑战。[Objective] To explore the intervention mode of HIV/AIDS prevention among high risk men based on clinical service of venereal disease. [Methods] Follow-up study was conducted. Male patients who sought diagnosis and treatment service in venereal disease clinic services and had experience of seeking sex service were provided 3 months clinical diagnosis and treatment and services combined with behavior intervention. The questionnaires about behaviors were investigated before and after intervention. [ Results ] After intervention, the accurate recognition rates for knowledge about route of transmission and easy obscured knowledge among high risk men increased by 9.2% and 50%, respectively ; The rate of frequent condom utilization among these men who had sex with fixity sexual partner increased form 56.1% to 70.7%, while the rates of frequent condom utilization among these men who had sex with non-fixity sexual partners and female commercial sex workers increased by 2.1% and 15.5%, respectively. [Conclusionl The combination mode of clinical diagnosis and treatment and intervention is effective to improve the HIV related knowledge among high risk men. In the further, we should focused on the universal of knowledge about non-transmission and prevention measures. The behavior of condom utilization intended to be a better trend. Clients withdraw is the one of the challenges for a clinical research.
