摘 要:目的探讨西安地区健康体检人群血清总蛋白(TP)、白蛋白(ALB)的含量及球蛋白/白蛋白(A/G)比值的参考值水平。方法采用回顾分析的方法,随机抽取1000例在我院例行健康查体人群,用日立7600-020全自动生化分析仪进行血清TP、ALB的测定,并计算A/G比值,按不同年龄、不同性别进行分组统计分析,并与温州、贵阳、沈阳、河南焦作、杭州地区人群进行比较,观察血清蛋白质参考值的变化。结果西安地区健康体检人群血清TP67.7-84.1g/L,ALB38.8-56.4g/L,A/G1.00-2.36,且随年龄、性别及居住地区不同存在着差异,健康体检人群血清TP、ALB水平比原参考值明显偏高,A/G比值比原参考值明显偏低(P<0.01)。成年男性血清TP65.4-85.0g/L,ALB35.7-56.5g/L,A/G1.16-2.0,成年女性血清TP62.5-84.9g/L,ALB34.4-55.6g/L,A/G1.07-2.03,男性明显高于女性,男女之间TP、ALB有极显著性差异(Ρ<0.01)。TP现调查值超限检出率(3.3%)明显低于原参考值(10.3%),ALB现调查值(2.1%)低于原参考值(12.7%),A/G现调查值(1.6%)低于原参考值(8.1%),超限检出率降低67%以上。西安地区健康体检人群TP与温州、贵阳、沈阳、杭州相比有极显著差异,ALB与贵阳、沈阳、河南焦作、杭州相比有极显著差异,A/G与温州、贵阳、沈阳、河南焦作相比有极显著差异(P<0.01)。结论西安地区健康体检人群血清蛋白质水平与原参考值发生了改变,原参考值已不能很好的适用临床和健康体检的需要,应科学、合理的建立地区性的蛋白质参考值范围。Objective To investigate the reference levels of serum TP, ALB and A/G ratio of the healthy crowd in Xi'an area. Methods The high-level automatic biochemical analyzer (HITACHI 7600-020) was used to detect the level of serum TP, ALB and A/G of 1000 cases obtained randomly from the normal physical examination in our hospital and divided it into different groups by age, gender, and area, analyzed by the retrospective investigation. Results The value of TP ,ALB and A/G of persons' serum in Xi'an are 67.7-84.1 g/L , 38.8-56.4 g/L and 1.00-2.36 respectively. There is a difference exsiting because of age, sex and area of residence, the level of serum TP and ALB of healthy people higher than the original reference value, value of A/G lower than the original reference value (P〈0.01). The value of serum TP about adult males is 65.4-85.0g/ L, ALB 35.7-56.Sg/L, A/G 1.16-2.0 and the value of serum TP about adult females is 62.5-84.9 g/L, ALB 34.4-55.6 g/L, A/G1.07-2.03.From these data we can see that the males' value is higher than that of women and there is a very significant difference on TP, ALB between women and men (P〈0.01). The overrun detection rate of TP now (3.3%) is lower than the original reference value (10.3%) ALB (2.1%) lower than origin(12.7%), A/G (1.6%) lower than origin(8.1%) and overrun detection rate has been reduced more than 67%. The TP value of healthy crowd in Xi'an has a very significant difference with that of Wenzhou, Guiyang, Shenyang, Hangzhou; the value of ALB is very significant different from that of Guiyang, Shenyang, Henan Jiaozuo, Hangzhou; the value of A/G is also a very significant different from that of Wenzhou, Guiyang, Sbenyang , Henan Jiaozuo (P〈0.01).
