摘 要:目的了解外科手术对于凝血系统的影响。方法以正常人作对照,我们对16例胃大部切除的病人在手术前后的凝血、凝血因子和凝血酶生成(TG)做了分析。结果术前术后,PT有显著延长,fⅦ的显著性降低;fⅧ升高了2倍,但APTT和fⅤ并未有显著改变;TG的测定中最大凝血酶浓度没有显著改变,凝血酶的总量是有所降低的,而达到10nM的凝血时间却缩短了。结论凝血系统可以通过凝血酶的快速形成实现对手术引起的凝血活化及血液流失后的补偿,从而维持凝血系统的稳定性。Objectives To explore the changes of coagulation system induced by operation. Methods Thrombin generation (TG), prothrombin time (PT), activated partial prothrombin time (APTT), factors Ⅴ, Ⅶ and Ⅷ were determined in plasma samples of 16 patients before and after the subtotal gastrectomy. Results Comparison of pre-and post-operative results showed significantly delay of PT, significantly decrease of fⅦ, 2- fold increase of fⅧ, but no change of APTr and fⅤ. Although the maximum concentration of thrombin did not change, the total quantity of thrombin (ETP) was decreased with a shorten lag time to 10 nM. Conclusion The activation of coagulation system induced by operation takes advantage of the fast formation of thrombin.
