鼻内镜下鼻中隔成形术的可行性研究

摘　　要：目的探讨鼻窦内窥镜下鼻中隔成形术的可行性。方法2003年6月—2005年5月对对我科收治的100例鼻中隔中、重度偏曲患者采用平卧位,在鼻窦内窥镜直视下进行鼻中隔成形术。结果100例鼻内镜手术组总有效率97%,与100例Killian手术进行对比,后者总有效率67%,两者比较,x2=30.49,P<0.01,统计学差异显著;再与100例坐位前鼻镜下鼻中隔成形术进行对比,后者总有效率95%,两者比较,x2=0.52,P>0.05,统计学差异不显著;但是,鼻内镜手术组平均手术时间33’±12’,前鼻镜手术组平均手术时间50’±13’,两组比较,t=2.833,P<0.01,统计学差异明显;鼻内镜手术组术中平均失血量21±15ml,前鼻镜手术组术中平均失血量30±18ml,两组比较,t=2.712,P<0.01,统计学差异明显。结论鼻窦内窥镜下鼻中隔成形术是一种良好的手术方式,其疗效满意、出血少、手术时间短、安全可行,优于其他的鼻中隔矫正性手术,值得临床推广应用。Objective To explore the feasibility of septoplasty under nasal endoscope. Methods Between June 2003 and May 2005, 100 patients with moderate and severe deviation of nasal septum in supine position underwent septoplasty through nasal endoscope in our department. Results The total effective rate of nasal endo- scope in 100 patients was 97% and Killian＇ method 67%. There was a statistical difference between the two methods （x^2=30.49, P〈0.01）. Compared with the total effective rate （95%, in 100 patients） of anterior rhinoscope in sitting position, there was no statistical difference （x^2=0.52, P〉0.05）. However, the average operation duration of nasal endoscope was 33＇±12＇ and anterior rhinoscope 50＇±13＇. There was a statistical difference between the two groups （t=2.833, P 〈0.01）. The average blood loss of nasal endoscope was 21± 15ml and anterior rhinoscope 30± 18ml There was a statistical difference between the two groups （t=2.712, P〈 0.01）. Condusion Septoplasty under nasal endoscope has some advantages including satisfactory outcome, less bleeding, shorter operative duration and safety. It is better than other surgical methods for nasal septum deviation and worth extending in clinical practice.
