镜下清理、刨削、髌外侧减压术治疗膝关节骨性关节炎42例

摘　　要：目的:探讨关节镜下清理、刨削、髌外侧减压术对膝关节骨性关节炎的治疗效果.方法:42例患者根据临床症状、X线片以及髌骨倾斜试验的结果,采用关节镜下清理、髌外侧支持带松解术.结果:所有病例切口I期愈合.平均随访28 mo,劳累后膝部胀痛4例;屈曲受限30°左右2例;4例术后2 a症状复发,行全膝关节置换术.其余病例无明显膝部疼痛,膝关节伸曲功能正常.Lysholm评分:术前(41±7)分,术后(85±5)分(P<0.01).结论:膝关节骨性关节炎患者行膝关节镜下清理、髌外侧支持带松解术,可明显缓解膝部疼痛,提高生活质量,效果满意.AIM： To explore the therapeutic efficacy of arthroscopic debridement and lateral patella retinacular release on knee osteoarthritis. METHODS： Forty-two patients with knee osteoarthritis, according to clinical symptoms, radiographic results, patellar inclination test and gliding test, were treated with arthroscopic debridement and lateral retinacular release of the patella. RESULTS： All patients obtained primary wound healing. Within follow-up of 30 months on average, 4 patients felt pain after fatigue. Two had the knees motion range limitation for about 30 degree. Four had total knee replacement because of symptom recurrence. Others had no obvious pain or abnormal motion of the knee. Lysholm knee score was increased from 41 ± 7 preoperatively to 85 ±5 postoperatively（P 〈0.01 ）. CONCLUSIONS： Arthroscopic debridement and lateral retinacular release of the patella can be an effective surgical procedure to alleviate pain of the knee in patients with osteoarthritis. Preoperative patellar condition should be evaluated thoroughly.
