人工流产术后与正常经期放置曼月乐避孕效果比较

摘　　要：【目的】比较曼月乐(左炔诺孕酮宫内释放系统)在人工流产术后即时放置与正常经期放置的避孕效果。【方法】对74例自愿选择放置曼月乐的经产妇分人工流产术后即时放置组(观察组)和正常经期放置组(对照组),观察组39例,对照组35例,随访12个月,对其避孕效果和副反应进行观察对比。【结果】观察组与对照组的妊娠率均为0,脱环率分别为2.56%、0,续用率分别为94.87%、94.28%,两组各项相比无显著差异(P>0.05)。副反应(闭经、异常出血、体重增加、痤疮、乳胀)发生率低,在两组间相比无显著差异(P>0.05)。【结论】曼月乐在人工流产术后即时放置避免了二次宫腔操作及短期内再次妊娠,具有与正常经期放置相同的避孕效果,是人流术后较理想的避孕选择。[Objective] To compare the contraceptive effect of Mirena （Levonorgestrel-releasing intrauterine system, LNG-IUS） insertion immediately after artificial abortion and in the period of regular menstruation. [Methods] Seventy- four multiparas inserted with Mirena were divided into two groups. Thirty-nine muhiparas were inserted right after the artificial abortion nominated as observing group, and thirty-five muhiparas inserted in the period of regular menstruation as the control group. Follow-up was performed one year afer insertion. Contraceptive effect and side effects were compared between two groups. [Results] No pregnancy was found in both groups. The expulsion rate were 2.56% and 0% respectively, and continuation rate were 94.87 % and 94.28 % in the observing group and control group respectively. There were no significant difference between two groups （ P 〉 0.05）. No significant difference was found in side effects （amenorrhea, abnormal bleeding, weight gain, acne and swollen breasts） which were low between two groups （P 〉 0.05）. [Conclusions] Secondary intrauterine surgery and pregnancy again in a short period are avoided by Mirena insertion immediately after artificial abortion, which share the same effect with insertion in the period of regular menstruation. Mirena is considered to be a good choice for contraception after artificial abortion.
