摘　　要：目的探讨吡格列酮对糖耐量减低合并非酒精性脂肪肝患者血清脂联素变化的影响。方法吡格列酮组46例糖耐量减低合并非酒精性脂肪肝患者，在饮食、运动治疗的基础上，于早餐前15min口服吡格列酮30mg／d，治疗16周后进行分析比较血清脂联素水平变化，治疗前后比较用配对t检验，治疗后组间比较用独立样本t检验。结果糖耐量减低合并非酒精性脂肪肝患者采用吡格列酮治疗16周后与治疗前相比血清脂联素水平明显升高（t=9．14，P〈0．01），胰岛素抵抗指数明显下降（t=5.10，P〈0．01）；吡格列酮组与对照组治疗16周后相比血清脂联素水平升高明显（t=7．63，P〈0．01），胰岛素抵抗指数明显下降（t=5．37，P〈0．01）。结论吡格列酮治疗后，糖耐量减低合并非酒精性脂肪肝患者血清脂联素水平明显升高，改善了胰岛素的敏感性，降低了胰岛素抵抗。Objective To study the effect of pioglitazone on serum adiponerctin levels in patients with impaired glucose tolerance combined with non - alcoholic fatty liver. Methods On the basis of dietotherapy and kinetotherapy ,the 46 cases took pioglitazone（30 mg/d）15 minutes before breakfast for 16 weeks. Before and after the treatment of pioglitazone, the levels of serum adiponerctin were determined to compare. Then made analysis of paired samples t - test for the before and after treatment, and made analysis of independent sample t - test for the treatment group. Results There was distinct rise on serum adiponerctin levels in the patients with impaired glucose tolerance and non -alcoholic fatty liver after treatment with pioglitazone for 16 weeks （t = 9.14, P 〈 0.01 ） ,and distinct decline of the index of insulin sensitivity （ t = 5.10, P 〈 0.01 ）. Contract with the control group, there was distinct rise of the serum adiponectin levels in pioglitazone group after treatment for 16 weeks（ t = 7.63, P 〈0.01 ）, and distinct decline of the index of insulin sensitivity（ t = 5.37, P 〈 0, 01 ）. Conclusion After treatment with pioglitazone, the levels of serum adiponerctin rose distinctly in patients with impaired glucose tolerance combined with non -alcoholic fatty liver. It improved the insulin sensitivity and reduce the insulin resistance.
