摘 要:目的:探讨致肾盂肾炎大肠杆菌毒力因子的分布及其与耐药性的关系。方法:用PCR和多重PCR方法检测81株尿源大肠杆菌的毒力因子基因papC、fimH、hly、cnf1和aer等;用K-B法进行15种抗菌药物的敏感试验。结果:papC阳性41株(50.6%),为致肾盂肾炎大肠杆菌(UPEC)组,papC阴性40株(49.4%),为非UPEC组。UPEC组hly、aer、fimH、cnf1的检出率依次为63.4%、61.0%、53.7%和34.2%;非UPEC组依次为10.0%、35.0%、12.5%和5.0%;2组之间4种毒力因子的检出率差异均有统计学意义(χ2分别为24.746、7.977、10.281和9.085,P<0.05或P<0.01)。总耐药率为87.65%,UPEC组和非UPEC组的多重耐药率之间差异有统计学意义(P<0.01),前者(87.8%)明显高于后者(50%)。结论:UPEC菌株毒力因子检出率高,多重耐药性突出,应引起临床上的高度重视。Objective: To study the distribution of the virulence factor and its relationship with the antibiotic resistance of the pyelonephritic E.coli(UPEC). Methods: The virulence factors,papC,fimH,hly,cnfl and aer gene of the 81 strains of E.coli isolated from urine were tested using PCR and multiple PCR. The sensitivity tests of 15 drugs were detected by K-B methods. Results: The positive rate of the papC was 50.6% in UPEC strains. The negative rate of the papC was 49.4% in non-UPEC strains. Among the UPEC strains, positive rates for hly,aer,fimH and cnfl were 63.4% ,61.0% ,53.7% and 34.15%, respectively. Among the non-UPEC strains those positive rates were 10.0%, 35.0%, 12.5% and 5.0% respectively (χ2 = 24.746,7.977,10.281 and 9.085, P 〈 0.05 or P 〈 0.01 ). Drug-sensitivity result showed that the total resistant rate was 87.65%. There were statistical differences between the UPEC strains( 87.8% ) and the non-UPEC strains(50% ) in multiple antibiotic resistant rates (P 〈 0.01 ). Conclusion: It is important to pay attention to the higher positive rate of UPEC and serious multiple antibiotic resistance clinically.
