颈动脉斑块与冠状动脉斑块形态的关系

摘　　要：目的:探讨冠状动脉斑块与颈动脉内-中膜厚度(CCA-IMT)及斑块指数的相关性。材料和方法:收集经冠状动脉脉造影确诊为冠心病患者82例,根据斑块形态分为三组,即Ⅰ型病变(表面光滑)组31例,Ⅱ型病变(表面不规则)组32例和Ⅲ型病变(长段不规则)组19例;另设冠脉造影正常的25例为对照组。采用高频超声探测双侧颈动脉,观察IMT及有无软、硬斑。结果:冠心病组颈动脉CCA-IMT厚度、软斑PI、硬斑PI、总斑PI高于对照组(P<0.01);冠心病Ⅱ型病变组软斑PI、总斑PI明显高于Ⅰ型、Ⅲ型病变组(P<0.01);而Ⅱ型病变组CCA-IMT、硬斑PI与Ⅰ、Ⅲ型病变组相比无差异(P>0.05);Ⅰ型病变组与Ⅲ型病变组相比各组指标无统计学差异(P>0.05)。结论:颈动脉斑块(特别是软斑)与冠状动脉斑块的形态有一定的相关。Purpose： The aim of this study was to investigate if intimamedia thickness（ IMT）, plaque index of the carotid artery related to the coronary artery lesions morphology in patients with coronary heart disease. Materials and Methods： Eighty-two patients with coronary heart disease （CHD） and twenty-five controls underwent coronary angiography. Based on the morphologic features of the plaque, the patients of CHD were divided into type Ⅰ （ n = 31, with smooth interlumenal surface）, type Ⅱ （ n = 32, with irregular surface）, and type Ⅲ （ n = 19, elongated lesions） groups. The IMT and plaque indices of carotid artery in those patients were assessed by high frequency ultrasonography. Results： Compared with normal controls, CCA-IMT, the soft and hard plaque indices, total plaque index in patients with CHD were significantly higher （P〈0.01）. CCA-IMT and hard plaque indices in Type Ⅱ CHD did not significantly differ to that in Types Ⅰ and Ⅲ （P 〉0.05）, but the soft plaque and total plaque indices did. No difference on CCA-IMT, soft and hard plaque indices, total plaque index was found between Type Ⅰ and Type Ⅲ （ P 〉 0.05 ）. Conclusions： A certain relation existed between angiographic morphology of the coronary artery lesion and senographic features of carotid atherosclerotic plaque in patients with coronary heart disease.
